These organizations were previously known as Local Health Integration Networks (or "LHINs") at a time when they also led local health care planning and funding, and those functions are now part of Ontario Health.
Home and Community Care Support Services South East is dedicated to ensuring the ongoing delivery of local services while Ontario makes changes to improve the health care system to give patients better-connected care with health care providers working as one coordinated team in Ontario Health Teams.
The south east region extends from Brighton on the west, to Prescott and Cardinal on the east, north to Perth and Smiths Falls, and back to Bancroft. The boundaries are for funding and planning purposes only. Position Summary

As a Clinical Support and Education Coordinator, you will provide training and education on clinically based assessments, organizational policy and procedure, clinical processes and practices, and service planning standards for HCCSS. Your role will also be involved in clinical utilization analysis and ensure that the organizational standards of care are designed to meet patient needs, are evidence based fit within target utilization/scope of service, and meet applicable legislation standards.

What will you do?
Clinical Support & Utilization Activities
- Analyze Patient population care needs and service utilization
- Review currently implemented service planning standards to evaluate effectiveness, identify issues/gaps, and monitor progress toward standard practice
- Design and develop service planning standards for specific Patient populations based on care requirements and evidence based practice
- Conduct audits (regular and ad-hoc) to evaluate compliance with existing service standards, coding and CHRIS/Clinical Business Processes.
- Work with Home and Community Care operations to understand variance from developed standards and devise strategies for improvement
- Support the development of Policies and Procedures related to Patient care and service planning
- Assists in identifying improvement opportunities and the selection and prioritization of improvement initiatives
- Participates in projects to develop and implement new programs and program improvements, following organizational project management processes and standards
- Coordinates status reporting and tracking of patient services projects and initiatives.
- Actively participate in designing the change management strategies associated with the implementation of new programs
- Participates in the implementation of standard service and utilization tools and processes
Clinical Education
- Complete clinical education needs assessments and develop learning outcomes, course outlines and curriculum;
- Plan, deliver and evaluate all assessment tool training (i.e., core, remedial, recipient and refresher);
- Assist with the sustainability of CHRIS, interRAI and related standardized Patient record processes across Home and Community Care Support Services South East (business change process review and training);
- Support projects to develop and/or implement tools for audit and evaluation of Client service staff uptake, proficiency and accuracy with Home and Community Care Support Services South East client service management systems;
- Develop documentation standards and guidelines with respect to interRAI outcomes, CAPS and clinical service planning;
- Develop strategies for implementation of standardized assessment tools, coding and outcomes for evidence-based clinical and operational decision making
