**Current Rate of Pay**: Min $42.91- Max $53.63

**Current Shifts**: Primarily Days (approx. 6 months)

**Qualifications, Experience, Skills and Abilities**:

- Valid registration with a Professional Healthcare Regulating Body
- Completed Baccalaureate Degree; Masters preferred
- Minimum 3-5 years acute care hospital experience, preferably in a medical/surgical area.
- Project Management experience an asset
- Knowledge and experience with Long Term Care strategies, Alternate Level of Care (ALC) designation and discharge processes
- Must be able to determine capacity in accordance with the Health Care Consent Act (1996)
- Employee must demonstrate basic computer skills and literacy
- Ability to work with a diverse stakeholder group with demonstrated facilitating and coaching clinical areas on change initiatives
- Self-directed, courageous, and highly motivated with excellent interpersonal and communication skill
- Experience managing competing demands in a fast-paced, multi-disciplinary health care
- Evidence of effective communication and conflict resolution skills with the ability to develop positive working relationships with all stakeholders promoting collective problem solving approaches;
- Excellent interpersonal skills
- Demonstrated ability to work independently and in a self-directed manner as well as collaboratively within a team in a fast-paced and ever-changing environment
- Demonstrated commitment to excellent customer service when interacting with patients, families, colleagues and physicians
