**Details**:Full time, Days, 37.5 hours per week, Monday to Friday, 8:00 AM to 4:00 PM

**Pay Scale**:Commensurate with education and experience

**Location**:Central Services, 14220 - 109 Avenue NW, Edmonton, AB, T5N 4B3 (travel between various GEF sites is required, mileage paid)

The Outreach Support Worker (OSW) will report to the Community Support Manager and be responsible for developing and delivering GEF's Community Supports Programs to enhance the overall well-being of the residents and tenants living in the GEF communities. The OSW will work closely with seniors to identify their service needs and collaborate with other community support agencies and social service providers to help seniors access necessary services and resources.

**Typical Duties**:

- Develop and deliver GEF's Community Supports Programs to enhance the well-being of residents and tenants living in GEF communities.
- Work with seniors to identify their service needs and develop a plan to help them access necessary services and resources.
- Liaise with other community support agencies and social service providers to ensure seniors receive comprehensive support.
- Work as a part of a collaborative Client Services Team to support seniors in accessing necessary services and resources in their community to improve their quality of life.
- Provide ongoing support to seniors by monitoring their progress, making necessary adjustments to their service plans, and ensuring they receive timely and effective support.
- Maintain accurate records of service delivery and ensure all necessary documentation is completed in a timely manner.

**Experience/Education Preferences**:

- Diploma in Human Services, Social Work, Human Ecology
- 2 - 4 years’ work experience is an asset
- Experience working with Seniors, families, or groups is considered an asset
- Strong English communication skills, both written and verbal are required
- Strong computer skills in Microsoft Office and Case Management Systems is required

**Other Requirements**:

- Must have a vehicle and valid driver’s license and submit a copy of the driver’s license upon request
- Strong customer service skills including the ability to interact with everyone in a respectful manner
- Ability to identify and support seniors with a variety of challenges, barriers and service support needs
- Strong planning and organizational skills
- Strong negotiating and conflict resolution skills with a flexible and collaborative approach
- Ability to meet deadlines and respond calmly to stressful situations
- Must be able to work with mínimal supervision and maintain confidentiality
- Ability to work independently and as part of a team
- Ability to adapt to different environments
