The Lower Outaouais Family Health Team (LOFHT) is looking for a dedicated permanent full-time Primary Health Care Health Promoter. The LOFHT serves our community by promoting wellness and healing through collaborative care, where the team works together to support active participation of the patient towards their wellness goals. Located in Hawkesbury (half way between Ottawa and Montreal) our 24 family physicians and interdisciplinary health professionals work collaboratively to provide the best primary care to our 19,000 patients.
**Main Responsibilities**/ Position Focus**:
- Collaborates and/or consults with physicians and other interdisciplinary team members to provide high quality primary care
- Weight management program
- Smoking cessation program
- Chronic disease management programs
- Community-based exercise program
- Exercise guidance group and individual
- Participation on FHT Quality/Communication Committee
**Job Requirements**
- Bachelor degree in kinesiology, member of College of Kinesiologists of Ontario.
- Experience in exercise prescription with clients living with a chronic disease.
- Strong interpersonal skills to ensure effective communication with patients and team members, demonstrate ability to work in a team environment, demonstrate multi-tasking skills and ability to organize workload are essential.
- At least two (2) years clinical experience will be considered an asset
- Certification in CPR (BLS-C)
- Provide Vulnerable Sector Check
- Must be bilingual, oral and written

**Salary**: $69,335.00-$74,148.00 per year
