We are currently seeking an RPN who enjoys making a difference in the lives of others while caring for their client in the community with innovation and creativity in planing care, and treating their client in the client's private home. **Essential Job Functions/Responsibilities**:

- Provides direct client care as defined under the scope of nursing practice developed by the College of Nurses of Ontario while using sound judgment and critical thinking.
- Develops and implements a plan of care and treatment.
- Provides accurate, pertinent, detailed, and timely documentation consistent with the plan of care and the nursing process.
- Conducts ongoing client assessments, update care plans, follow up interventions, and evaluate outcomes.
- Participates in health/therapeutic counseling, teaching, and emotional support to the client, family, and caregivers.
- Observes client condition and accurately reports to the Director of Care or Nurse Manager.
- Assesses and provides client, family, and caregiver education and information pertinent to diagnosis and care.
- Prepares, administers, and appropriately documents actions taken specific to commonly prescribed oral, rectal, topical, subcutaneous, and intramuscular medications. Observes, reports and records adverse reactions from medications and/or treatment to the Director of Care.
- Recognizes the need for and institutes emergency measures in a rapid and appropriate manner, when indicated.
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (i.e., PT, OT, SW, and Dietitian) to the Director of Care.
- Performs all skills, treatments, and procedures competently in accordance with the provincial Nursing scope of practice and adheres to the policies, procedures, and protocols of the Agency.
- Follows principles of Infection Control and Universal Precautions.
- Uses equipment properly and in a safe manner. Reports broken equipment for repairs.
- Ensures client's rights are adhered to. These include but are not limited to:

- the right to privacy and dignity while care is being provided
- the right to accept or refuse care
- the right to the confidentiality of their information

**Qualifications**:

- Graduate of a Certified Community College Program for Registered Practical Nursing or University for BScN for Registered Nursing.
- Currently licensed and in good standing with the Ontario College of Nurses.
- Negative TB test or chest x-ray. (completed within the past 12 months)
- Clear Vulnerable Sectors Police Record Search (completed within the past 12 months)
- WHMIS Certified
- CPR & First Aid Certified
