In this pivotal role, you will draw upon your team building and change management skills to lead multi-disciplinary teams in advancing organizational priorities including the provision of excellent patient and family partnered care. You will be responsible for the seamless integration of new models of patient care delivery while ensuring that roles, responsibilities and functions continue to evolve to meet changing corporate needs. You will be accountable for the quality of patient care, utilization of resources, professional staff development and the day to day management of the inpatient unit Transplant/Nephrology and Urology and the Kidney Transplant Clinics. You will support performance monitoring and use of agreed upon metrics to evaluate success of the strategies being utilized. You will support your teams to adapt course as needed to meet corporate and program related objectives.

**DUTIES & RESPONSIBILITIES**:

- Being responsible for clinical, financial and human resources management with a focus on quality outcomes for Kidney and Metabolism patients in an inpatient unit and outpatient Transplant Clinics.
- Promotes best practices and evidence based care through the implementation of initiatives that are aligned with strategic directions of the organization
- Drawing upon your team-building and change management skills to advance clinical practice
- Being accountable for the development, implementation and management of quality improvement initiatives to ensure best possible patient care and outcomes
- Supporting organizational strategies and staff initiatives that promote the development of a motivated, cohesive team through educational support strategies to enhance staff knowledge, professional nursing practices and patient care
- Being responsible for staff development and for developing team-based approaches to the day to day management of the unit
- Responsible for ensuring that roles, responsibilities, functions and care delivery models continue to evolve to meet changing patient/family, departmental and corporate needs
- Demonstrating a patient/family focused approach to all care practices
- Working in collaboration with physicians to ensure the effective and efficient delivery of patient care services
- Effectively introducing and managing change within the units and supporting staff through the changes
- Developing and fostering positive internal and external relationships with clients, community partners and internal and external service providers
- Participating actively in program specific and hospital wide initiatives through committee and task force membership, hospital planning and management forums
- Develops approaches to support integration and team work within and between teams within the program

**QUALIFICATIONS**:

- Regulated Health Professional with Master’s Degree in Nursing/Health Discipline required
- Registration in good standing with a regulatory college
- Minimum five (5) years of clinical experience in working with patients and families in both an in and outpatient setting, Nephrology experience is an asset.
- Strong leadership skills including demonstrated effectiveness with designing, planning, managing and change management.
- Experience with labour relation issues. (e.g. grievances, investigations, counseling)
- Experience in facilitating, collaborating and negotiating effective resolution of human resource issues
- Demonstrated ability to contribute to a positive learning culture and facilitate the achievement of evidence based quality care with optimal outcomes
- Demonstrated human resource management skills including recruitment and retention of staff
- Proven experience developing and implementing education plans for various team members
- Strong analytical and conceptual skills with the ability to analyze and utilize clinical and management data for planning purposes
- Demonstrated excellent budgeting and financial management skills
- Demonstrated collaboration with internal and external stakeholders to develop, manage and evaluate programs/services considering client needs, service delivery inter-relationships and service potential within a complex environment
- Demonstrated excellent interpersonal, superior communication, coaching and team building skills that facilitate positive interactions among all members of the health care team
- Demonstrated ability to contribute to a positive learning culture and facilitate the achievement of evidence based quality care with optimal outcomes
- Demonstrated strong commitment to patient safety, best practice and innovation in patient care delivery
