**Job Purpose**

Under the direction of the Settlement and Family Support Director, the Case Management and Crisis Counsellor facilitates a case management team approach that will bring a team of professionals, service providers, programs and individuals to support the client through contribution of resources and expertise. This position ensures that action plans are carried out and that assessments are consistent and appropriate, team consultations and coordination are enhanced, and duplication is avoided.

**Duties and Responsibilities**
- Administration_
- Verifies/determines eligibility and gathers the necessary information and documents through Assessment /identifying client needs /strength and goal setting with clients, reviewing the assessment and other relevant documents.
- Organizes case management team meetings and facilitate group discussions with other professionals and service providers.
- Keeps supervisor informed by regular reporting as required and advising them on emergent issues.
- Provides program information and data by maintaining client records, preparing required reports, and participating in staff meetings and internal committees.
- Transfers/closes the case and ensure follow-up and support the client.
- Case Management_
- Provides settlement services to refugees and immigrants through facilitation of integrated case management.
- Assists immigrants in resolving complex cases by assembling integrated case management team, and ensures settlement by developing case management plans, providing crisis intervention and remedial or preventative measures.
- Monitors adherence to case plans and manages the flow of accurate information within the action system.
- Ensures the case plan, intervention, and progress is properly implemented, progress tracked and documented. Evaluates the case plan and follows up, following through with deadlines and tasks.
- Manages cases by assisting immigrant families and individuals to access community services through home visits, outreach activities, providing information and orientation, consulting, making contacts, making appropriate referrals, accompanying on appointments, following up with contacts, advocating, documenting case files, translating and interpreting for clients, and sharing client information through case consultations.
- Provides counseling to help the client and his/her family, supported with follow ups.
- Relationship Management_
- Identifies resources, refers clients to outside agencies. Maintains credibility and good public relations with formal and informal support systems.
- Acts as a liaison between the clients’ families, programs, other service providers, and professionals to ensure smooth flow of information and support.
- Fosters equitable treatment of immigrants by keeping up-to-date on policies and programs affecting immigrant settlement; and maintaining good working relationships with community service providers.
- Ensures service quality and collaborative work relationships by providing guidance to new staff and acting as a resource person in an identified area of expertise.
- Promotes respect for cultural diversity by informing and educating the community about immigrant and cross-cultural issues; and sitting on external committees.
- Other duties as assigned._

**Education and Experience**
- Bachelor’s degree in social work (preferred), psychology or education and one year of work experience providing counselling services and case management, OR
- Graduate of postsecondary program (2 year minimum) in the social sciences or humanities or education, and, two years of work experience providing counselling services.
- Cross
- cultural counseling experience required.
- Specialized training in program delivery area is an asset.
- Training in life skills counselling not mandatory but an asset.

**Required Licenses and/or Certifications**
- Criminal Occurrence Security Check - Must complete a Criminal Record and Vulnerable Sector Check in good standing and within three months prior to first day of employment.
- Proof of English proficiency by obtaining a completed Canadian education, Minimum CLB level 8, or IELTS 6.5.
