**The Agency**

**The Program**

The Program Navigator will function as part of a larger Adult Community Services Team. This dynamic and interactive team provides an array of programming and services targeting adults are or have been criminally involved. A reintegration services model encompasses programming which includes emergency intake and assistance, employment services, and community re-entry support; additional services offered include literacy, and digital literacy. All programming offers client-centered, strength based, trauma informed services and support. The team works collaboratively to provide effective and coordinated services to all clients.

**The Position**

The Program Navigator will be integral to the effective functioning of all of the Adult Community Services Team and the organization. The position involves client intakes for most of the Adult Community Services Team programs along with some ongoing Case Management for individuals who are not currently involved in a case management program utilizing a client-centered case management approach. This client-centered case management approach includes: Creating client-centered case management plans, navigating relationships with natural supports, goal setting, crisis resolution, accessing addiction and/or mental health counseling, accessing financial support, basic needs, securing affordable and safe housing, cultural connections, systems navigation, and creating positive lifestyle opportunities. Practicing client centered case management, the Program Navigator will build supportive community relationships; complete casework documentation; do data collection; and report outcomes.
