**As our new Resident Support Coordinator, you will**:

- Have a degree in Social Work or a Social Service Worker diploma.
- Maintain current registration with the Ontario College of Social Workers & Social Service Workers.
- Ensure effective safety procedures are developed and managed to comply with statutory obligations, in conjunction with relevant organizational policy.
- Coordinate and support the transition of residents moving into our Village, provide guidance in areas such as loss, life transitions and the Resident Bill of Rights.
- Support resident and family meetings, care conferences, therapeutic groups, and engage with outside agencies and community groups as needed.
- Document appropriate resident information and participate in the development of comprehensive plans of care.
- Provide mediation, education, training, coaching and counselling for residents, families and team members regarding mental health and well-being, while serving as a key representative for Resident and Family Council.
- Support residents on the waitlist and ongoing assessment needs with residents and those waiting for services.

**It would be an asset if our new Resident Support Coordinator has**:

- Current Heart Save Level of CPR
- Knowledge of relevant policies, procedures, legislation and regulations applicable to the long-term care sector, including the Fixing Long-Term Care Act, 2021 and Health & Safety Act of Ontario.
- Demonstrated experience working with people living with cognitive or physical impairments.
- Experience working with a geriatric population.
