The Alzheimer Society of Muskoka (ASM), serving the communities of Muskoka and Parry Sound, is dedicated to improving the quality of life for individuals living with Alzheimer’s disease and other dementias and their caregivers. We are committed to providing advocacy, a caring and understanding support system, individual, family and group counselling, public education.
The Alzheimer Society of Muskoka offers a wide range of programs and services addressing the progress of Alzheimer disease with support for caregivers and families including supportive counselling, public education, weekly drop in programs, in home recreation, a resource lending library, music and Minds in Motion.
**JOB SUMMARY**
This new exciting and dynamic position is a unique career opportunity to make a difference in the lives of those affected by dementia in our community. The Recreation Support Facilitator will create recreational-based intervention plans and provide information and supportive counseling interventions that meet the needs of clients in the stage of their dementia journey.
The Recreation Support Facilitator is responsible for developing and facilitating the Support Group Respite program to provide a safe and stimulating environment for persons living with dementia to allow Care Partners to be able to attend Support Groups.
In partnership with the Community Engagement Coordinator and In Home Recreation Coordinator, the Recreation Support Facilitator is responsible for screening, training, and supervising program volunteers and post-secondary student learners who are supporting support services program delivery.
**KEY RESPONSIBILITIES**:
- Good knowledge of ASM Mission, Vision and Values
- Compliance with Privacy Policy; obtains and records consents
- Develop, facilitate, maintain and evaluate Support Group Respite program
- Assist with coordination and facilitate recreational based programs as needed (ie. Drop
- In, music therapy, art therapy, etc.)
- Provide information and supportive counselling to care partners and persons living with dementia through office visits, home visits (when necessary), telephone consultations, virtual meetings (Zoom/OTN) while following Support Services policies and procedures
- Maintain client files/caseload and statistical data in accordance with ASM funding and policy requirements
- Working with In Home Recreation Coordinator to support the creation of recreation intervention plans for volunteers to execute with the matched client using a holistic approach and recreation domains, while providing activities consistent with the individual’s cultural diversity, recreational interests, and activities of daily living
- Support and process In Home Recreation client referrals and support the matching process with appropriate volunteer and family
- Develop and implement an annual Recreation Support Facilitator work plan
- Maintain current knowledge of dementia and relevant community resources
- Provide leisure education for care partners and other health care professionals involved in client case
- Update In Home Recreation Volunteer Training as new education, research and knowledge is gained
- Execute In Home Recreation Training to volunteers monthly and as needed through the support group respite program
- Manage a caseload of volunteers who are matched with clients
- Orientate, train and supervise volunteers who are matched with clients
- Serve as a liaison to the families, providing them with information about the Alzheimer Society of Muskoka and its services, and directing them to the appropriate supports
- Ongoing communication and monthly meetings with the Executive Director
- Collaborate with peers of ASiO and work closely with broader Society program staff for ongoing assessments of clients for recreational based programs
- Conduct regular program evaluations by soliciting feedback from clients, volunteer and ASM program staff
- Always represent the Alzheimer Society of Muskoka in a professional manner and act in the best interests of the organization
- Liaise with community partners in conjunction with the program, education, and fund development staff
- As an employee of a non-profit organization, you are expected to participate in fund development activities as required
- Other related duties as requested including occasional facilitation of the Minds in Motion program
**QUALIFICATIONS**
- Post-secondary degree/diploma in Therapeutic Recreation and/or Recreation and Leisure, Social Work or gerontology
- Minimum 3 years experience working with persons with dementia and/or their families
- Comprehensive knowledge of and understanding of Alzheimer’s Disease and other related dementias
- Strong group facilitation skills and experience
- Experience building relationships with volunteers, staff, and community partners
- Training and public speaking experience
- Experience working with cli
