**Position Description**

The Clinical Coordinator plays a key role in supporting the health and wellness of children and families across the First Nations communities served by the Kenora Chiefs Advisory (KCA). This position ensures access to culturally safe, community-based pediatric care by coordinating services across local clinics, partner organizations, and KCA’s own programs and teams.

Working in collaboration with KCA Primary Care Teams and the Nibiising Bimaadiziwin (NBDS) developmental services program, as well First Nations community Health Directors, the Coordinator ensures that children’s health services are integrated, holistic, and responsive to community priorities. The role also liaises with external partners such as Firefly, WNHAC, and Lake of the Woods District Hospital (LWDH) to support wraparound care and system navigation.

Position Title: Clinical Coordinator (Pediatrics)

Department: Primary Care

**Salary**: $50,000-60,000

Term: part-time, permanent (3 days/week) with potential to transition to full-time

Location: 240,Veteran’s Drive ,Kenora

Reports to: KCA Medical Director for operational tasks and Nursing Manager for Time Sheets

**Duties and Responsibilities**:
**Clinical Care Coordination**

Ø Coordinate pediatric services within First Nations community clinics, including scheduling clinics within communities for pediatric clinical assessments, overseeing distribution and completion of any pre-consultative questionnaires, coordination of referrals and follow-ups, and assisting with aspects of discharge planning at times to ensure seamless transition back to community and various other administrative tasks as needed.

Ø Work with the NBDS program, Firefly, and KCA pediatrician on coordination of pediatric assessments and administrative tasks as required to support referrals and case management of children with complex needs, ensuring culturally grounded, interdisciplinary care approaches.

Ø Collaborate with the KCA Medical Director, and the rest of the KCA Primary Care Administration and Teams, including physicians, to assist with coordination of schedules coverage of time off, to ensure care continuity and shared planning.

Ø Promote and support traditional wellness practices alongside western medical care, as guided by community protocols and Elders.

**Community & Family Engagement**

Ø Build strong, trust-based relationships with families, caregivers, and community members.

Ø Serve as a liaison and advocate for families, helping them navigate health services and understand care options where appropriate.

**Partner & System Collaboration**

Ø Coordinate with external service providers, including Firefly, LWDH, Rural Generalist and ANHP Recruiters and regional specialists, to support referrals, joint care planning, and follow-up.

Ø Participate in interagency meetings and collaborative care discussions as needed to support shared clients.

Ø Act as a key contact for pediatric specialist and care team.

**Leadership & Program Support**

Ø Support the development of service pathways, protocols, and resource materials related to pediatric care and primary care.

Ø Contribute to quality improvement initiatives and the expansion of pediatric and primary care programming within KCA.

**Documentation & Reporting**

Ø Maintain clear and confidential records related to coordination, follow-up, and family support.

Ø Assist in the collection of data and metrics for reporting to funders, leadership, and community partners.

Ø Contributes to evaluations of the pediatric care program and other KCA health initiatives.

**Qualifications**:
**Education & Experience**:
Ø Allied health care professional or strong background in health administration. Experience with pediatrics and primary care are assets.

Ø 3-5 years’ clinical experience, ideally within rural, Indigenous, or community health settings.

Ø Registered and in good standing with a professional college (e.g., CNO).

Ø Experience working with interdisciplinary teams and navigating multiple service systems.
