**Role Description Summary**

The Advanced Practice Clinician (APC) plays a key role in delivering high-quality, patient-centered care within Infini Health’s multidisciplinary healthcare environment. This position blends clinical expertise, advanced assessment skills, and a commitment to collaborative care to support optimal patient outcomes. The APC will independently assess, diagnose, and manage patients within their scope of practice, while working closely with physicians, nurses, and allied health professionals to ensure continuity and excellence in care delivery.

**Duties & Responsibilities**:
**1. Clinical Care & Patient Management**
- Conduct comprehensive health assessments, including patient history, physical examination, and diagnostic interpretation.
- Formulate differential diagnoses and develop evidence-based treatment plans in alignment with organizational protocols and patient preferences.
- Prescribe therapies and interventions within the regulated scope of practice.
- Perform advanced clinical procedures as authorized by licensure and training.
- Provide patient education on diagnosis, treatment, prevention, and wellness strategies.
- Monitor, evaluate, and adjust care plans to support ongoing patient needs.

**2. Collaboration & Interdisciplinary Practice**
- Work in partnership with physicians, registered nurses, and additional healthcare team members to deliver coordinated, integrated care.
- Participate in multidisciplinary case reviews, quality improvement initiatives, and care planning meetings.
- Support the integration of clinical best practices into daily operations and patient care pathways.

**3. Leadership & Education**
- Mentor and provide clinical guidance to nursing staff, students, and support personnel.
- Participate in professional development, ensuring up-to-date knowledge of emerging treatments, technologies, and clinical guidelines.
- Contribute to the development and implementation of clinical policies, protocols, and standards of care.

**4. Documentation & Compliance**
- Maintain accurate, timely, and comprehensive clinical documentation in accordance with regulatory and organizational standards.
- Ensure compliance with applicable legislation, regulatory requirements, and Infini Health policies.

**Qualifications & Skills**
- Current registration and licensure in Ontario or equivalent regulated Advanced Practice Clinician designation.
- Minimum of 3-5 years of clinical experience in a primary care, specialty, or multidisciplinary healthcare setting.
- Demonstrated competence in advanced health assessment, diagnostics, and therapeutics.
- Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification (or willingness to obtain).
- Strong clinical skills, with the ability to assess and respond to patient needs effectively.
- Excellent communication and interpersonal skills.
- Ability to work both independently and collaboratively in a team-oriented environment.
- Flexibility to adapt to varying schedules and clinic needs.
- Availability to work evenings and weekends as required.
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and familiarity with Electronic Medical Record Systems (EMRs) is a plus.
- Ability to uphold and maintain a high level of confidentiality in handling sensitive and confidential information.
- Working knowledge of healthcare administration, including regulations, compliance, and industry best practices.

**Working Conditions**
- Possibility of working evenings and weekends (Saturday's & Sunday's) to accommodate clinic needs.
- The role requires a degree of autonomy, strong communication, and collaboration within a dynamic, high-volume setting.

- The role may involve physical activities, including long periods of standing, administrative duties, lifting or transferring patients, and moving medical equipment.
