**Job Summary**
The Case Manager provides high-level expertise in supporting vulnerable youth and families, independently managing complex caseloads across shelter, housing, and resource programs. This role develops individualized service plans, resolves multifaceted challenges, and collaborates with internal teams and external partners to ensure youth have access to the resources, stability, and relational support needed to thrive. The Case Manager also coaches other staff, contributes to program development, and upholds trauma-informed, client-centered, and Christian faith
- aligned care.
**Responsibilities**
- Manages a diverse caseload of youth and/or families, conducting assessments, goal planning, and crisis intervention with clients ages 12-24.
- Develops individualized service plans addressing needs in housing, health, mental wellness, employment, education, legal advocacy, and life skills.
- Coaches less experienced staff, volunteers, and practicum students on relational care, documentation, and youth engagement best practices in close collaboration with their manager.
- Evaluates client progress regularly, making adjustments to case plans and connecting youth with internal and external support services as needed.
- Collaborates with other team members, funders, and community partners to ensure coordinated care and seamless referrals.
- Resolves complex situations through safety planning, conflict mediation, and trauma
- informed crisis management.
- Maintains accurate case notes, reports, and outcome tracking in accordance with program requirements.
- Supports program transitions by working across age-specific and housing-based services to ensure continuity of care.
- Advises leadership on trends, gaps in services, and opportunities for program enhancement.
- Represents Cyrus Centre at manager approved community events, committees, tables and tasks groups.
- Provides on-call shift coverage to support the continuity of youth support and services.
- Contributes to the cleanliness, upkeep, and overall atmosphere of the facility
- Perform other duties as assigned by supervisor within the same scope and level of the role.
**Skills & Abilities**
- Advanced case management and crisis intervention skills
- Strong communication, documentation, and interpersonal abilities
- Ability to work independently while contributing to a collaborative team environment
- Knowledge of trauma-informed care, harm reduction, client-centered care, and housing
- first principles
- Skilled in de-escalation, mediation, and complex problem-solving
- Strong relational approach grounded in compassion and consistency
- Professional presence in partnering with families, service providers, and external agencies
- Ability to work and operate within the Microsoft Office 365 suite of products.
**Qualifications**
- Bachelor’s degree in Social Work, Child & Youth Care, Psychology, or a related field (or
equivalent experience)
- 4-6 years of relevant experience supporting vulnerable youth and/or families in a community or housing context
- Experience in case planning, systems navigation, and collaborative service delivery
- Emergency First Aid and Non-Violent Crisis Intervention training are assets
- Valid Class 5 driver’s license required
- Successful completion of a Criminal Record Check (vulnerable sector)
- Alignment with the mission and Christian values of Cyrus Centre
**Job Types**: Full-time, Permanent
Pay: From $25.00 per hour
Expected hours: 32 - 40 per week
