Summary:
Reporting to the Coding Coordinator, the individual is responsible for coding and abstracting health records according to established guidelines, using computerized systems, and ensuring compliance with national, provincial, and regional standards. They process corrections to abstracts to ensure data quality, maintain the computerized abstracting system, and engage in continuous quality improvement activities. Additionally, they provide data for specific projects and assignments as needed, assist in presenting statistical information, and contribute to organizational committees by offering expertise on codes, classification, mapping, and data collection.

Graduate of an approved Health Information Management Program. Canadian College of Health Information Management (CHIM) Certified and active membership with the Canadian Health Information Management Association (CHIMA).

**Your day to day**:

- **Coding and Abstracting**:

- Perform coding duties for databases (CIHI, Trauma Registry, Neonatal Network, etc.) as per guidelines.
- Ensure accurate and complete data capture for hospital statistics and research.**Data Quality and Accuracy**:

- Review patient records for accurate documentation.
- Ensure the quality of data and resolve discrepancies.**Data Definitions and Design**:

- Participate in developing data definitions and modeling for health records.**Period End Functions**:

- Review patient files for proper coding and abstraction.
- Verify system accuracy and resolve any issues.**Quality Improvement**:

- Suggest improvements to the Coding Coordinator.
- Participate in implementing necessary changes.**Data Reporting**:

- Collect and analyze data for specific projects.
- Provide reports to relevant departments or committees.**Support Analysts**:

- Act as a coding expert for internal/external analysts.
- Provide guidance and respond to coding-related queries.**Clinical Mapping and Data Activities**:

- Assist in structured data activities, including clinical mapping.**Policy Input**:

- Participate in reviewing and suggesting updates to policies and procedures.
- Assist in creating and revising forms.
