**Park Time Social Worker**

Summary of Position
- Providing Social Work intervention from a team based model to all McCormick Dementia Services clients and caregivers in the form of assessment, individual family and group counselling, crisis intervention, case management, discharge planning and making appropriate referrals to other health care providers and social service agencies

Background/Qualifications
- Bachelor or Master’s Degree in Social Work required
- Successful registration with the Ontario College of Certified Social Workers and Social Service Workers required
- Minimum of 3 years’ experience in the long term care or community support service sector
- Experience in counselling /supporting clients with dementia and their families/caregivers
- Experience in dealing with issues around client transitions and adjustment (eg. admission, discharge, transfer, change in health status and function, decline in independence, etc.).
- Experience in facilitating and leading support groups
- Knowledge of relevant community support services
- General knowledge of the Public Guardian and Trustee Act, Powers of Attorney and Advanced

Care Directives
- Ability and knowledge to work within a computerized environment
- Excellent organizational, interpersonal and communication skills
- Ability to work with all levels of the organization
- Demonstrated commitment to relationship-centred care
- Must be able to perform all duties utilizing the guiding principles of McCormick Care Group: Compassion, Accountability, Respect, Excellence, and Collaboration

**Job Types**: Part-time, Fixed term contract

Pay: From $35.67 per hour

Expected hours: No less than 15 per week
