London Health Sciences Centre (LHSC) and St. Joseph?s Health Care London (SJHC) are looking for a qualified Stroke Navigator to join our two organizations in a unique shared model within the Clinical Neurosciences Program (LHSC) and Rehabilitation Program (SJHC).

Reporting to the Manager, Clinical Neurosciences LHSC and the Coordinator of Rehabilitation Services at SJHC, the Stroke Navigator will assess stroke client needs and facilitate transitions to the most appropriate services that will meet the individual and functional goals of the client, from referral to discharge environment and community re-integration. The Stroke Navigator works as a member of the stroke health care team, and in conjunction with partners within London Health Sciences Centre (LHSC), St. Joseph?s Healthcare London (St. Joseph?s) Parkwood Institute Main Building (PIMB) and other community partners (e.g. LHIN).

The Stroke Program navigator is a position that supports a comprehensive model of care for stroke patients. Services include CNS, Emergency Department, Interventional Radiology as well as the Rehabilitation programs at PIMB (or other programs at PIMB as required). The stroke model is designed to ensure that clients receive optimal care across the continuum in keeping with stroke best practices.

Temporary Full-Time

Rate of pay**:$48.19 per hour - $60.23 per hour

Hours of work: 37.5 hours per week

Duration: July 2026

**Qualifications**:

- Successful completion of a four year Undergraduate Degree in a Health Care related field
- Current certificate of registration from a college recognized under the Regulated Health Professions Act
- Current BLS for Healthcare Providers course: BLS-HCP(C) preferred
- Minimum 3 years recent, related experience
- Advanced clinical knowledge and expertise in stroke care across the continuum
- Demonstrated clinical experience in neurological rehabilitation, behaviour management, adult learning principles, consultative/collaborative approach to stroke recovery
- Advanced knowledge of program algorithms and processes (i.e. Algorithm for Priority of Patient Pulls, In patient tPA algorithm, Priority Access to Carotid Intervention Algorithm, site wide algorithms (i.e. Bed Management Algorithm, Gridlock Protocol, Bed Map Protocol)
- Proficient knowledge of LHIN, regional, provincial and national health care systems and the interaction of these systems within the program
- Demonstrated strength in interpersonal relations and communication skills, both verbal and written
- Proficiency with computer skills including the use of Microsoft Office (Word, Excel, Outlook), PowerPoint presentations
- Demonstrated organization skills, problem solving, critical analysis and flexibility
- Demonstrated excellent planning and evaluation skills
- Demonstrated excellent change management skills
- Demonstrated project management abilities
- Demonstrated leadership skills, critical thinking, emotional intelligence
- Demonstrated knowledge of and commitment to the principles of patient and family centred care
- Demonstrate knowledge of and commitment to patient and staff safety at LHSC
- Demonstrate ability to attend work on a regular basis
