Christie Gardens seeks a Registered Practical Nurse to work as full time Clinical Support Coordinator. This position will provide support to the nursing team throughout the continuum as well as assist the Director of Health and Wellness with care planning and service delivery. **The schedule for this position is Monday - 7am-3pm, Tuesday-Friday - 11am-7pm.**

**Qualifications**
- Registered Practical Nurse with at least two years’ experience in a Retirement or Home Care
- Experience in working with seniors with various cognitive and physical capabilities
- Excellent communication skills, verbal and written
- Caring and seeks to promote the dignity and integrity of individuals
- Ability to work independently and collaboratively within a team, be resourceful and demonstrates flexibility
- A mature attitude with a genuine interest in seniors

**Responsibilities**
- Assist with the maintenance of records reflecting residents’ condition, care, and specific treatment
- Complete and review Care Plans in a timely manner as per RHRA regulations
- Make daily rounds to help and support team
- Daily review 24 hour report and follow up as necessary with team
- Pharmacy communication and support as needed
- Complete audits as requested by Director of Health and Wellness and Team Leader
- Oversee clinical care processes to ensure that care is delivered according to individual plans of care for residents
- Lead the implementation and monitoring of required clinical programs, i.e. Falls Prevention, Skin and Wound Care, Continence Care, Pain Management, responsive behaviours
- Practice in accordance with CNO standards and support nursing team as appropriate
- Meet high standards of cleanliness and adhere to Infection Control and Prevention
- Accept responsibility for proper use and care of equipment
- Assume the duties of the Admin. Assistant as required
- Ensure confidentiality of all sensitive material and information
- Perform nursing duties to ensure clinical excellence at all levels in the care community
- Actively participates in committees according to the terms of reference and areas of responsibility
- Model and foster an environment of positive customer service in all aspects of care
- Maintain an active presence in the Resident Home Areas
- Mentor staff
- Oversee and provide support for Staff Education
- Ensure completion of the provision of quality resident care
- Regularly review, revise, develop and maintain the systems of identifying, recording and reporting of infections in residents. Maintain regular contact with all resident programs, and fully investigate all potential and actual infectious issues
- Lead and attend all meetings as required
- Use safe working practices and ensure responsibilities as per the Occupational Health and safety Act are performed
- Perform additional duties as assigned

**Salary**: $27.00-$28.50 per hour
