**Summary**
As a member of the Lighthouse Supported Living Inc. the Wellness and Addictions Counselor is an integral team member that provides supports to individuals that may be facing homelessness and/or addiction. The primary goal of this position is to encourage, facilitate, maintain and enhance recovery processes with people who struggle with addiction and/or addiction related issues. The Wellness and Addiction Counselor engages with residents, develops a helping relationship, implements various recovery modalities and accesses community services to the benefit of the resident. Responsibilities include providing informational services as well as individual and group counselling for residents. This position is expected to assess individual and group needs of residents and implement plans with them to pursue goals.

**Job Duties**
- Assertively engage residents and build a therapeutic relationship.
- Holistically assess resident needs and challenges, identify strengths and existing supports as well as assessing sources and level of motivation —as per assessment format.
- In collaboration with the resident and where possible with their existing support system, develop an
Individualized Service Plan (ISP) which will identify resident goals, resident strengths, available resources and an action plan.
- Meet with clients to assess their substance issues.
- Understanding their substance of choice, their triggers and what led them to seek substances.
- Identify recovery interference situations.
- Understanding situations that may cause a resident to increase and/or introduce new substances etc.
- Educate resident about coping mechanisms.
- Identifying unhealthy coping mechanisms in residents and educating/employing new & healthy mechanisms.
- Conduct both individual and group therapy sessions for residents.
- Providing one on one time for residents as well as group sessions in order to develop healthy relationships in a safe and respectful environment.
- Track and analyze resident recovery progress and revise plans as required.
- Update legal system as required.
- In certain situations, courts may order “conditions” for individuals. This may include meeting with an addictions counselor and/or abstinence etc. In these cases, regular updates to the legal system are required.
- Along with resident, develop a plan that is reasonable and achievable for resident as they transition through programming.
- Develop a patient crisis management plan.
- Assist resident in planning for potential crisis and coping mechanisms to help deter them from falling back on toxic habits and cycling.
- Intervene during emergency situations.
- Monitor clients and intervene as needed to ensure resident safety.
- Explore with resident and arrange for the management of acute physical withdrawal both from an immediate as well as longer term perspective.
- Identify existing supports and support network.
- Work closely and collaboratively with Mental Health and Addictions Services (SHA) as well as other service providers including COAST, MHSA, Detox and treatment centres.
- When appropriate and available, work with other Lighthouse staff to support transition to other
Lighthouse resources and services.
- Facilitate resident access to appropriate community services to address the resident’s housing/bio/psycho/social/spiritual/recovery issues and needs. Advocate for resident and help with admission processes and paperwork. Remain involved to ensure the connection was successful.
- Encouraging resident initiative and independence, work with them and their existing support system to plan discharges and support transitions.
- Communicate with relevant Lighthouse program teams to develop and execute plan to support as per their requirement.
- Contribute to and promote a community climate of respect, safety and mutual support among residents and staff of the Lighthouse.
