At Community Healthcaring Kitchener-Waterloo (The Health Centre), we provide a range of healthcare services aimed at improving the health and well-being of our community. We focus on providing care to individuals facing health inequities and barriers to accessing essential health services. Our team is made up of Family Physicians, Nurse Practitioners, Nurses, Medical Secretaries, a Chiropodist, and other healthcare professionals, all working together to make a positive impact.

**Your Responsibilities**
- Deliver exceptional care to complex clients, providing assessments, diagnoses, treatments, and health education as per the College of Physicians and Surgeons of Ontario standards.
- Provide consultation to Nurse Practitioners when needed, ensuring continuity and comprehensive care.
- Collaborate with team members to contribute to the development of medical protocols and health promotion programs.
- Engage in ongoing quality improvement initiatives, and participate in team and staff meetings to help us deliver exceptional care and meet organizational goals.
- Potential to contribute to outreach efforts to support community health and reduce health inequities.
- Participate in research projects and data collection to advance healthcare delivery.

**What We’re Looking For**
- A Medical degree from a recognized university and certification with the College of Physicians and Surgeons of Ontario (CPSO).
- Membership in the Ontario Medical Association (OMA) and the Canadian Medical Protective Association (CMPA).
- Experience working with diverse, vulnerable, and complex populations in an inter-disciplinary setting.
- Strong communication skills and the ability to work collaboratively with both healthcare professionals and clients.
- Proficiency with electronic medical records and MS Office Suite.
- A passion for working to address health disparities and improve access to care.
