The Wabano Centre for Aboriginal Health is an award-winning Health Access Centre that provides holistic health services to First Nation, Inuit and Métis people living in Ottawa - one of the fastest growing and diverse urban Indigenous communities in Canada. Wabano’s Seniors Team is an interdisciplinary 6-member team that has been providing culturally-appropriate care to Indigenous seniors for over 15 years. The team includes a nurse, case manager, coordinator and other community service workers. Working collaboratively, team members provide Indigenous seniors living in Ottawa with home visits, resources for basic necessities, accompaniment to medical appointments, cultural programs and activities, transportation and active outings.

**The Role**:
Reporting to the Seniors Team Lead, the Community Service Worker would work as part of the Seniors Team to provide home visits and resources for basic necessities.

As a Community Support Worker, you will take an active approach in providing assistance to urban Indigenous Seniors in maintaining self-control over their health. We’ll rely on you for wellness checks and being able to drive the seniors to appointments and program sites. You will also get the opportunity to collaborate with other team members in coordinating and planning activities and initiatives for the seniors.

To meet these responsibilities, the Community Support Worker will:

- Visit seniors in their homes
- Provide continual support and encouragement for seniors in their efforts to be independent in their home;
- Encourage positive changes as needed so seniors can remain in their home;
- Follow the care plan developed for the senior (by the Case Manager), observing and reporting any substantial findings or changes to clients’ overall physical, emotional, mental or environmental status to the Team Lead;
- Perform housekeeping duties at senior’s residences (dusting, vacuuming, cleaning kitchens and bathrooms, changing beds, organizing personal items);
- Prepare light meals as needed or requested by the client;
- Assist with shopping for groceries, household and personal items;
- Manage inventory of supplies;
- Accompany seniors to programs;
- Document in the Electronic Medical Record system in order to monitor client’s well being.

- Post-secondary completion in Social Services or other related field of study or an equivalent combination of education and experience;
- Demonstrated experience working with Indigenous seniors;
- Knowledge of social, economic and health concerns relevant to the urban Indigenous community;
- Proven ability to positively work with a team and independently;
- Strong organizational skills with evidence of oral presentation skills and written communication skills for report writing;
- Proficiency with Microsoft Office tools, particularly Excel, and Word;
- Ability to work flexible hours including evenings and weekends;
- Ability to build, foster, and maintain positive professional relationships;
- Excellent interpersonal communication skills;
- Valid Ontario Class “G” or equivalent license.
