The Home Programs and Nephrology Clinics is intended to enhance the patient experience through increased system efficiency and care coordination of patients through their healthcare continuum from the Multi-care Kidney Clinic to home modality of choice; home hemodialysis or home peritoneal dialysis. The program has been developed to meet the principles of access and consistency while providing quality care. The ultimate goal of the program is to provide care that is coordinated and navigated along a person-centred care continuum with all information available to the patient and their partner-in-care, minimizing the time intervals along the full continuum of care

DUTIES AND RESPONSIBILITIES:
Reporting to the Patient Care Manager, the Unit Coordinator will utilize the nursing process to provide safe, competent care to assigned patients. The unit Coordinator's primary role will be the facilitation, operationalization, and coordination of patient transitions to the home programs at LHW, coordination of nursing assignments and serve as a resources for all shifts, facilitating patient flow, and acting as a steward for developing and facilitating the patient-centred approach within the home programs and Nephrology Clinics. The Unit Coordinator will support home hemodialysis services including Home Dialysis Assistance Program (HDAP), are provided in compliance with Ontario Rental Network (ORN) funding and role requirements.

The Unit Coordinator will also assist in the support of any required data collection as per requirements of the Ontario Renal Network (ORN), to maintain data accountability in the Ontario Renal Reporting System (ORRS), assist and lead in quality and/or process improvement initiatives as determined by the Regional Nephrology Program. Contribute to and meet the deliverables and expectations of the ORN, quality improvement (QI) initiatives, focused on improving timely access to home modalities. Participate in ORN Home Dialysis teleconferences as required, share learnings and best practices on local QI initiatives related to home modalities and mentor new staff. Coordinate the delivery of quality care and education in accordance with the ORN defined clinical responsibilities.

This position may require some travel and flexible working hours to meet patient and unit care needs. The Unit Coordinator will be part of the nursing team and will be expected to cover for other home program nurses as required.

QUALIFICATIONS:

- Registration with the College of Nurses of Ontario (in good standing);
- Current BCLS;
- Baccalaureate degree in nursing, preferred;
- Recent Unit Coordinator/ Charge Nurse experience required (must be within the last 2-3 years);
- Minimum three (3) years' recent experience in home hemodialysis required;
- CNA certification in Nephrology or eligible to write and with the commitment to do so within one year of hire is preferred;
- Completion/In progress of de Souza Institute Patient Navigation Course (mandatory);
- Completion of continuing education in the area on Nephrology nursing;
- Knowledge and solid understanding of ORN strategic direction and priorities
- Experience with the development and implementation of effective education programs targeted at

patients and their partners-in-care;
- Proven ability to effectively and efficiently coordinate, implement and monitor the plan of care for each patient;
- Excellent communication skills with proven ability to collaborate with patients, partners-in-care, and

members of the healthcare team both institutional and community based;

- Recent & related supervisory experience is an asset;
- Proven understanding of staff scheduling (ONA and CUPE), staff/patient assignments and patient flow
