As a **Care Connector (Status: Temporary, Full-Time Start date: Immediately to October 24, 2025; Location: Any Ontario Health atHome office within the North East)**, helps patients navigate through a complex health care system by connecting people to an appropriate primary care practitioner (Unattached Patient Program - UPP).

Whether you work in our office, in the community, or a health care facility - you will play a lead role in providing connected, accessible, patient-centered care - and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.

**What will you do?**
- Collaborate with patients, their families, caregivers, or substitute decision
- makers to find Primary Care Provider within their community based on their health care needs.
- Work closely with individuals to understand their health care needs to make the best match possible.
- Provide information and referral assistance to patients to access alternative community resources.
- Evaluates patient satisfaction to identify trends and areas of improvement.
- Establish and maintain effective relationships with Community Care, local agencies, primary health care professionals, and other appropriate agencies.
- Accurately document and chart patient's assessment results, goals, treatment plans, and interventions.
- Balance patient needs and choices with available resources, ensuring patient’s values and preferences are respected.

**What must you have?**
- Registered Practical Nurse in good standing with the College of Nurses of Ontario;
- Good knowledge of primary care services and other community resources in Ontario Health atHome for the area of North East, as well as the roles of health care professionals;
- Relevant experience in a community health care environment;
- Practical knowledge and understanding of relevant legislation (e.g. regarding the provision of health care services, privacy, etc.);
- Effective planning, organizing, and evaluation skills to manage multiple clients, provide information reports, and take corrective action as required;
- Ability to accurately complete required documentation, reports, forms, and recommendations;
- Strong listening, interpersonal, facilitation and communication skills to work with diverse client groups with varying levels of comprehension and language capability;
- Ability to establish and maintain a wide range of contacts with health care professionals and community organizations;
- Advanced oral and written proficiency in English and in French is essential.

**What would give you the edge?**
- Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
- Case management experience or recent related community experience
- Ability to speak French or another second language
