Detailed Overview:
In accordance with the Mission, Vision and Values of Fraser Health, and Mental Health & Substance Use program mandates, the Clinical Coordinator works collaboratively within an interdisciplinary team-based model of care and promotes rapid engagement in the provision of consistent, relevant and timely delivery of client and family-centered care to the Urgent Care Response Centre (UCRC) and other programs as assigned. The UCRC is a community outpatient centre that supports adults in crisis with mental health and substance use concerns requiring urgent care but not hospitalization, and provides coordinated
- one-stop
- services including assessment, initiation of treatment, crisis stabilization management, and facilitation of connection to appropriate care services.

The Clinical Coordinator has responsibility for the day-to-day operations of the services including daily client/patient flow, supervises and provides work direction to staff, coordinates staffing coverage, provides leadership, guidance and support to team members and acts as a clinical resource to the interprofessional team. With an emphasis on recovery and rehabilitation, reducing barriers to enter the system of care, and promoting a culturally-safe and responsive, trauma-informed approach, establishes a welcoming therapeutic environment and promotes person-centred care, client self-determination and independence. Services may be provided on an outreach basis when required in order to assess and/or engage with clients within their own environment and community.

**Responsibilities**:

- Provides leadership to the inter-professional team by coordinating client care activities, providing information and consultation to designated care providers and students through methods such as reviewing client and family needs or progress and troubleshooting and providing guidance/knowledge in the management of a client. Supports the staff involved in the caring for clients and their families by providing information based on specialized theory/practice, assessing clinical skills and/or assisting staff with client care activities as required and provides leadership on best practices related to the care of high risk clients. Monitors the quality of care and the safety of patients. Provides assessments of clients as required. Promotes a positive team environment, and ensures that staff have access to critical incident debriefing.
- Supervises and provides work direction to nursing, allied health, Mental Health Care Workers and administrative support staff and coordinates the work of the clinical team overall on a day-to-day basis by methods such as scheduling, coordinating and adjusting work activities/assignments/schedules in accordance to the needs of the unit, establishing priorities, providing feedback on staff performance, and ensuring service levels and staffing requirements are met. Determines related training and orientation requirements and educational needs in collaboration with the CNE and provides input on setting and achieving team goals and objectives for the unit. Contributes as a team member to the selection of new staff and performance evaluation of staff through methods such as attending interviews, assessing clinical skills and providing input.
- Evaluates the provision of care to clients and their families in accordance with the organization vision and values by observing staff/students, reviewing/auditing relevant documentation, interviewing patients and families to investigate clinical components of incident reports and providing intervention as required.
