**Job Summary**

**Responsibilities**
- Conduct comprehensive assessments of clients' needs, strengths, and challenges.
- Develop individualized care plans in collaboration with clients and their families.
- Provide counseling and support to clients dealing with emotional, social, or financial difficulties.
- Advocate for clients' rights and access to necessary services.
- Maintain accurate and confidential client records in compliance with regulations.
- Collaborate with healthcare providers, community organizations, and other stakeholders to coordinate services.
- Monitor client progress and adjust care plans as needed.
- Participate in case management meetings and contribute to team discussions.

**Skills**
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent communication skills, both verbal and written.
- Empathy and active listening skills to understand clients' needs effectively.
- Problem-solving abilities to develop effective intervention strategies.
- Knowledge of social services resources and community programs.
- Ability to work independently as well as part of a multidisciplinary team.
- Proficiency in case management software and documentation practices.

**Job Types**: Full-time, Permanent

Pay: $57,000.00-$74,000.00 per year
