St. Joseph’s Hospice is a charitable, faith-based organization that offers compassionate holistic care and support to people living with a palliative illness. Support is extended to their family members and caregivers and to those grieving the loss of a loved one. Hospice programs and services are offered at no cost and are available irrespective of age, gender, religion, race, ethnicity or economic status.

St. Joseph’s Hospice is excited to recruit for the position of **Spiritual Care Practitioner**. We serve our residents, clients, and caregivers in our 10-bed residence and in the community. We also have an agreement to provide spiritual support to the London Middlesex Palliative Care Outreach Team.

The **Spiritual Care Practitioner **supports a variety of programs and services designed to promote a person-centred, seamless approach to care, supporting the Psychological, Spiritual, Social, Practical, End of Life Care/Death Management and Loss and Grief Domains of Care of individuals and their loved ones throughout the Palliative Care - Chronic Disease Continuum, including bereavement.

**Key Accountabilities include, but are not limited to**:
**SPIRITUAL CARE and COUNSELLING**:

- Through spiritual care assessments identifies, guides and facilitates the spiritual support and/or interventions for the unique faith needs of individuals in our Residential and Community Programs and the London-Middlesex Palliative Care Outreach Teams (PCOTs), and their identified circle of care.
- Builds strong relationships with various faith leaders to optimally address the diverse faith, cultural and spiritual needs of those we serve, communicating and facilitating connections with established Client faith supports, as appropriate;

**PROGRAM IMPLEMENTATION and EVALUATION**:

- In collaboration with members of the interdisciplinary team, provides input into the direction of spiritual and supportive care programming to meet the diverse needs of those we serve.
- Adheres to the theoretical framework for spiritual and supportive care service provision.
- Ensures services are:

- reflective of the Mission, Vision and Values of St. Joseph’s Hospice, and in alignment with our Strategic Plan; andevidence-informed and delivered in an effective and equitable manner.

**QUALIFICATIONS:
To perform this role successfully, an individual must be able to perform essential duties satisfactorily. The requirements listed below are representative of the knowledge, skills, and /or abilities required.
- University degree in Religious, Theological or Pastoral studies
- Master’s degree in counselling, Religious, Theological or Pastoral studies
- Certificate in Pastoral Care (Canadian Association for Pastoral Practice), or equivalent
- Member of Canadian Association for Spiritual Care (CASC) is preferred
- Certificate in Grief and Bereavement or Thanatology from a recognized post-secondary institution, preferred
- Certificate in the Fundamentals of Hospice Palliative Care, or the commitment to obtain
- Demonstrated relationship management skills consistent with the Mission, Vision, and Values of St. Joseph’s Hospice
- Willingness to work flexible hours and possess reliable personal transportation and be approved by insurance carrier to drive personal vehicle
