The Social Worker works in the home care setting, in person and virtual visits, as part of the CBI Home Health team serving Ottawa Community.

**What you need to be successful**
- You have a Bachelor of Social Work or a Master of Social Work
- You are registered as a member in good standing with the Ontario College of Social Workers and Social Service Workers
- Ability to travel from home to home within the community
- **a valid driver’s license, vehicle and car insurance is required**:
- Your focus is to embrace person and family-centered care
- You are known for your excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
- You value the role a therapist plays in keeping clients at home when that is of personal importance
