- Southbridge Care Homes are currently looking to add one a caring, passionate, and enthusiastic RAI Manager to our amazing home family, who can serve our lovely residents, to keep them safe, healthy, and happy. Reporting to the Director of Care, this position will assist with meeting MDS-RAI timelines and teaching staff about the requirements for data collection and reporting guidelines that support the Home’s funding._
**_ RESPONSIBILITIES _**
- _Coordinates the completion of MDS assessment on all residents within established timelines._
- _ Understands how the MDS assessment integrates with existing clinical tools and provides leadership in streamlining these processes._
- _ Understands and provides critical analysis of data at different levels of aggregation. (Resident, Home Health Zone, etc.)._
- _ Articulates for staff the strengths of MDS as a clinical assessment tool including how RAP triggers and care plans enhance quality care._
- _ Articulates the financial implications of coding inconsistencies._
- _ Acts as key contact and liaison with: Health Zone/AH&W/CIHI/PCC or others as appropriate on RAI data._
- _ Develops facility specific material within the PCC program as required._
- _ Locks/Unlocks the RAI individual assessments as required._
- _ Batches and submits assessments quarterly to appropriate data repository._
- _ Responds to Health Zone on all submission errors._
- _ Conducts and/or facilitates response and correction of submission errors with facility staff._
- _ Coaches key facility personnel to build capacity for understanding RUG patient classification levels. _
- _Develops and introduces processes, and systems to improve accuracy of RAI content._
- _ Conducts and/or facilitates audits of RAI assessments for content accuracy._
- _ Develops systems to decrease coding error rates._
- _ Participates in strategic planning for program development/improvement._
- _ Keeps current on RAI trends._
- _ Responsible for Clinical and Admin Setup role to support MDS within PCC software program._
- _ Interprets reports generated by Health Zone/AH&W or CIHI and provides critical analysis for Home._
- _ Understands the development of RAP’s, quality indicators, and RUG case mix products._
- _ Nursing Restorative Care. Facilitates the assessment of residents for nursing restorative Care, NRC. Inputs tasks in PCC software, educates Registered Staff on Care planning and evaluations related to NRC._
- _ Monitors and corrects expected performance of regulated staff_
- _ Other duties as assigned._
**_ QUALIFICATIONS:_**
- _ 3-5 years experience in Long Term Care as a licensed/regulated health care practitioner,_
- _ Experience with MDS an asset_
- _ Strong analytical and assessment skills_
- _ Ability to lead training and education with Adult learners_
- _ Knowledge of: InterRAI suite of tools, MDS/RUG III patient classification systems. _
- _Working Knowledge of: Project Management Principles, Industry Knowledge, Principles of Adult Education, Facilitation, Problem Solving, Continuous Quality Improvement Principles, Data Analysis, Microsoft Word, Excel, Outlook. _
- _Ability to read, write and communicate effectively._
- _ French language proficiency an asset._
