**About the Opportunity**:
As part of this project, the Operations Consultant will use an intersectional best practice approach to develop and refine the program for older adults and their caregivers living with chronic conditions. The program will be community-driven, leveraging self-management support approaches, and integrating research, best practice, and lived experience. The program development will have an equity, diversity, inclusion, reconciliation, accessibility (EDIRA) lens. The goal of the program is to support older adults and their caregivers to live their healthiest life and thrive.

**About You**:
We would love to hear from you if you have the following:

- Baccalaureate degree from a health discipline/health science field.
- Master’s Degree in Business Administration, Health Services Administration or equivalent combination of education and experience considered an asset
- Minimum of 3-5 years' working experience and responsibilities in a senior coordinator position or equivalent
- Canadian Evaluations Society's Credentialed Evaluator designation considered an asset
- A recognized training certificate in project management considered an asset
- Experience in planning, coordinating, delivering and evaluating PHC and community-based initiatives preferred
- Understanding of a trauma-informed approach
- Experience within a health-related environment preferred
- Experience in a primary health care/community-based setting preferred

**About You Continued**:

- Experience engaging with community
- Knowledge of disease prevention, health promotion, chronic disease management, and population health preferred
- Proven ability to work with a variety of stakeholder groups
- Demonstrated oral and written communication skills
- Valid Nova Scotia Driver's license and access to a reliable vehicle
- Competencies in other languages an asset, French preferred

**Hours**:

- Temporary Part-time (40%) position; 30 hours bi-weekly
- Approximately 6 months
- Shifts may include evenings, weekends and holidays.
- Travel within HRM will be required

**Compensation and Incentives**:
$36.33-45.41 Hourly
