**POSITION DESCRIPTION - MANAGER, HOME VISITING HUB (0.8 FTE)**

**Key Responsibilities - Clinic Operations and Management**

Utilizing their strong organizational skills, the Home Visiting Hub Manager will be responsible for overseeing the day-to-day operations and overall functioning of the clinic. They will do so through:

- Provision of leadership to the staff, addressing staffing issues and concerns.
- Staff supervision, performance management and evaluation.
- Assists with clinical and referral needs/questions.
- Management of physician coverage schedules for HVH and Hospice weeks of service.
- Coordination/arrangement of learners.
- Management of Quality Improvement projects.
- Acting as the primary point of contact for staff and physicians as issues arise.
- Acting as a liaison between the Home Visiting Hub and the Edmonton West Primary Care Network.
- The provision of technical support to physicians and staff as needed, becoming an EMR super user.
- Assisting/supporting recruitment of physicians.

**ATTRIBUTES/QUALIFICATIONS**

The Home Visiting Hub Manager must be knowledgeable of/can:

- Understand the palliative care supports available to patients and families in the community, and the role HVH plays within the larger system.
- Demonstrate sound judgement and critical thinking skills to assist in problem solving for staff and physicians.
- Understand Quality Improvement initiatives, including data collection, analysis, and presentation.
- Develop, maintain, and update organizational processes/procedures.
- Maintain relationships with multiple stakeholders and ensure clear communication among the team.
- Have strong organizational skills to manage multiple projects at a time.

**KEY RESPONSIBILITIES**
- Abides by and demonstrates PHS’s Mission, Vision and Values through both behavior and job performance daily.
- Complete all staff and physician scheduling, including last minute requests for time-off.
- Liaises with institutions and individuals who are seeking learning opportunities - tracks requests, schedules learners with physicians and ensures all paperwork is completed by learners prior to starting.
- Liaises with the PCN as required.
- With input from the Physician Lead, Education and Innovation leads all QI projects, including determination of project priorities, ensuring correct data is being collected, informing team of project, monitoring progress, and analysis and presentation of data to the team and stakeholders.
- Conducts quarterly panel reviews with physicians to ensure clients remain appropriate for program, physician panels meet contractual requirements.
- Assists in physician recruitment and onboarding.
- Supervision of HVH clinic staff (medical office assistant and clinic nurse).
- Attends and participates actively in Interdisciplinary Team Meetings, staff meetings, educational in-services, and actively contributes to quality outcome measures.
- Supports physician requests.
- Ensure filing of documents and files is kept up to date in a timely manner.
- Reviews and adheres to all company policies and procedures as outlined in the Employee Handbook and Policy and Procedure Manuals.
- Participates in special projects and performs other duties as required.

**SKILLS AND ABILITIES**
- Excellent written, verbal, and interpersonal communication skills.
- Outstanding organizational skills and the ability to manage multiple priorities.
- Demonstrated ability to foster and maintain a collaborative work environment.
- Able to work effectively both independently and as part of a team.
- Manages conflict with diplomacy and tact.
- Advanced computer skills: MS Office Suite, Electronic Medical Records systems.

**REQUIREMENTS**
- Applicants with a bachelor’s degree in nursing and active registration with CARNA are encouraged to apply.
- Minimum 2 years of supervisory experience.
- Physician management experience is considered an asset.
- A combination of relevant experience and education will be considered.
