Health Promoter

**Term**: Permanent Part Time - 15-22.5 hours per week

**Job Summary**: The West Carleton Family Health Team is a multidisciplinary high paced family medicine practice. We are looking for a Health Promoter that is experienced in a primary care environment.

The WCFHT **Health Promoter** will lead, support and advise on all Health Promotion activities associated with WCFHT.

**Position Goals**:

- Incorporate the health promotion to include all WCFHT clinical programs.
- To concentrate, track and be the lead for health promotion activities at WCFHT
- To effectively and equitably enable patients and staff to achieve control over self-care and lifestyle issues
- To engage externally with other sectors and organizations, including work places and schools, in creating lifestyle improvement initiatives.
- To enable the development of health promotion plans and activities relating to important disease categories, clinical teams, geographic/community/neighborhoods
- Develop and maintain an inventory of health related activities in the region.
- To develop practical and effective metrics suitable for measuring self-care progress for individuals, sub-populations, and programs
- To effectively involve patients in planning efforts
- Collaborate with committees to assist in providing one-on-one, group and family-based counselling as required
- Make it easier for patients to find resources that promote a good lifestyle.
- Update/refresh the information we make available to our patients. Keep current.
- Health Promotion expert for the external website (built on Weebly), waiting room TV, exam room screens, quarterly newsletters, mass messaged, bulletin boards, Pamphlets, Social Media, etc.
- Completion of tasks and duties delegated by the Executive Director or delegate;
- Travel as necessary to meet patients, community partners, etc.

**Required Skills**:

- Previous experience in program/service development and implementation;
- Strong understanding of principles of behaviour change, knowledge transfer and evaluation;
- Demonstrated ability to work within a multidisciplinary team and independently;
- Demonstrated presentation and creative problem solving skills;
- Excellent written and oral communication skills in English;
- Multi-media skillset, including basic technical skills for settings up virtual meetings, conference rooms, etc.
- Excellent organizational and time-management skills and ability to prioritize workload;
- Ability to work under pressure on several projects concurrently and continue to meet deadlines;
- Ability to establish and maintain positive working relationships with key stakeholders;
- Ability to facilitate or lead committee meetings;
- Access to a personal vehicle for travel

**Preferred Qualifications**:

- Previous experience in program implementation in health-care/wellness settings;
- Previous experience in a Health Care setting.
- A degree or diploma in Health Promotion or a related field

**Hours of work**
- The schedule of work will be determined when you are hired (Monday-Friday). You may be required an occasional evening (to 8PM) Monday to Thursday.

**Compensation**

Compensation will be determined based on Education and Experience.
