Ajax, Canada Durham Family And Cultural centre Full time

**The Family and Community Outreach Worker** will support and enhance family well-being through community-based prevention and early intervention services anchored for Black and racialized parents, children, guardians and or caregivers living in Durham Region. The incumbent will work full time, 35 hours per week, including some evenings, from our location in Ajax, as well as virtually, and report to the Executive Director.

**Responsibilities**:

- Provide navigational supports to Black and racialized parents and families and their children to identify their needs and goals.
- Advocate on behalf of families and assist in accessing resources and skills development/training to build capacity to self-advocate.
- Collaboration with community partners and participants to develop systems advocacy
- Provide short term therapeutic counselling support including crisis intervention as needed.
- Maintain strong documentation practices, consult with other members of the care team as necessary, and actively participate in clinical and peer supervision.
- Plan and facilitate group programming to meet the unique needs of clients served.



**Case Management **
- Participates in and/or leads regular case conferences to ensure good communication regarding the needs of the client and to promote care planning and improvements in the quality of service provided.   
- Develop connections with service providers in other agencies and institutions in which the client is involved to facilitate comprehensive case planning. 
- Consultation with other staff and health professionals as required in establishing a plan of care as needed.

- Maintains supportive relationships with clients and family members. 
- Complete monthly Program reports and ministry required reports
- Maintains up to date knowledge of community resources.

**Advocacy and Community Development **
- Provide case advocacy to ensure that clients receive appropriate community and health supports.
- Participates in community projects or activities to provide education, information and/or to develop collaborative strategies.

**Qualification Requirements**:

- Master’s degree in social work (or related field) from an accredited University and a minimum of 3 years successful experience in directly related field; or equivalent education and experience
- Access to a vehicle and a valid full G Class Ontario Driver’s License with appropriate insurance is required.
- Experience in working with diverse cultures and communities. A thorough understanding of anti-oppression, anti-Black and Indigenous racism values and principles and an appreciation of and ability to work with diverse communities.
- Solid knowledge of services and resources available for families in the Durham Region.
- Strong Case management skills
- Highly effective written and oral communication skills, problem solving, advocacy and assessment skills.
- Excellent time-management skills and ability to multi-task and prioritize work
- Ability to work collaboratively and independently
- Experienced in addressing compassionate matters and maintaining confidentiality

Pay: $67,000.00-$70,000.00 per year
