Primary Worker Support

**About Our Job**

Murray's Millwood Manor is hiring for a full-time Support Worker to Case Manager. This position performs standard performance professional level social work services including case management, counseling, referral, placement, and assessment /evaluation on less complex assignments and receives supervision on more complex assignments.

This position will be located at 409 Mill St., Kitchener. Interested applicants must be willing to work in a secure setting and available to work non-traditional shifts and workdays.

Job responsibilities include:

- Individual meetings with clients to address their individual responsivity needs
- Monitoring of clients during structured time
- Documenting individual and group client interactions
- Modeling pro-social behaviors and providing on-going behavior change feedback to clients
- Completing assessments as determined by the client’s individual treatment needs
- Discharge planning with client and assigned case management staff
- Participating in and supervising clients during community outings

**About You**
- Have experience working in Community Corrections or with a Criminal Justice Involved Population as well as with homelessness prevention programs and HFIS
- Strong computer skills
- Knowledge of research and interviewing techniques sufficient to be able to determine what information is needed, secure and analyze desired information, and formulate logical recommendations
- Consistent attention to detail and the ability to multi task
- Commitment to working collaboratively within a team to ensure work is completed timely
- Motivated to work with high risk/high need clients and ability to demonstrate a healthy working alliance with clients to assist them with behavior change and risk reduction
- Degree in counseling, social work or related field

Education: Bachelor's Degree in Social Work, Psychology, Sociology, Guidance and Counseling, or a related field.

Experience: One (1) year of experience as a professional social case work experience at the type and level of Social Case Worker.

Education & Experience Equivalency: No substitution of experience for education is permitted.

Additional appropriate education may be substituted for experience requirements.

Licenses and certifications must be kept current as a condition of employment

Full-time hours: 35 per week
