Reporting to the Social Worker
- Coordinator, the Tenant Resource Specialist is responsible for providing support to clients and their families. This includes but not limited to, coordination of client admissions in conjunction with the site Interdisciplinary Team and AHS Case Manager, orientation to the facility, interpretation and follow-through of client and family needs, referral of client and/or family intervention as required.

**KEY ACCOUNTABILITIES (INCLUDE BUT NOT LIMITED TO)**:

- Responsible for planning, developing, and implementing services to meet the emotional and social needs for clients in the facility.
- Leading the new admission process for all new intakes
- Provide therapeutic clinical counseling services to clients and families in the facility.
- Conduct social work and psycho-social assessments using a variety of modalities and in accordance with professional standards and clinical policies and practices and educate and provide consultation to clients, families, and healthcare professionals.
- Participate on the interdisciplinary team in care planning for the effective utilization of client resources and participate as a member of local committees and planning bodies to identify areas for improved efficiency and effectiveness in service delivery to improve client care services and reduce gaps in the community.
- Provide education to client/family/interdisciplinary team as needed regarding Social Work practice.
- Communicate with community health and social agencies including government ministries, Continuing Care personnel and family physicians by collaborating in the assessment and management of individuals at risk, making referrals to appropriate resources when longer term treatment or follow-up is required and exchanging relevant information.
- Record social work data and chart involvement with clients, assessments, interventions, progress, and discharge notes.
- Consistently demonstrates regard for the rights and responsibilities of clients and their family or substitute decision makers in matters such as privacy, beliefs, values, respect, and confidentiality.
- Responsible for point in time education and evaluation materials
- Leading new admission and family orientation sessions
- Coordinates and serves on the various committees responsible for client care.
- Co-leads the Client Council meeting on a monthly basis.
- Completes Annual & Mandatory Education/In-Services as well as all required education/in-services as per organizational requirements.
- Other duties as assigned

**EDUCATION AND EXPERIENCE**:

- Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
- Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation.
- Experience within the past 5 years working as a Social Worker in a Mental Health/Harm Reduction environment
- Experience within the past 5 years working as a Social Worker in Supportive Living.
- Organization skills, creativity and high motivation are required.

**WORKING CONDITIONS**:
This industry requires empathy for and an understanding of the needs of seniors and requires a complete and current (within six months) **Vulnerable Sector Check including a Criminal Background Check.** Fast paced, challenging, working environment that provides/coordinates complex services to seniors and other marginalized populations. The ability to be flexible to meet the demands of this work environment is required for success. An understanding of Addictions and Mental Health and Trauma Informed care, in a Harm Reduction philosophy is an asset. This work can be stressful, and you may encounter challenging behaviors including physical or verbal aggression. This position does require shift work.
