STR8 UP is hiring a fulltime Outreach Workers for the Community Intervention Model in Saskatoon, SK

**About STR8 UP**

STR8 UP is all about assisting and supporting addictive gang members in their transformation from dysfunctional and fatal relationships, to healthy and fulfilling relationships with self, others, Mother Earth and Higher Power, based on spiritual values of Honesty and Humility.

**Purpose and Scope**

STR8 UP’s outreach support adults and youth in correctional facilities and the community as they move through the justice system and transition out of the gang lifestyle to become responsible community members. They face major socio-economic and health challenges, systemic racisms, addictions and individual challenges. Outreach advocates, mediates, counsels, mentors and negotiates resources on their behalf. Must be living a healthy lifestyle.

**JOB SUMMARY**:The Outreach Worker is part of a wrap around team that provides support and consultation to the Primary Outreach Workers. The Outreach Worker will be responsible for supporting and implementing case plans, and assisting with all aspects of development and delivery of programming. The OW provides support to Primary Outreach Workers and service providers in developing participant’s release (if applicable) and monthly service plans. The OW will interact with participants, help identify and establish appropriate community supports, as well as coordinate administrative aspects of case management. The delegated Primary Outreach Worker in consultation retains final decision-making responsibility.
- Additional duties may include:_

Keep meticulous intake, case planning and attendance and referral records

Advocate in justice, corrections, social services sectors and for community supports

Provide one-to-one life skills and problem-solving counselling

Accompany and support members attending important appointments

Assist in obtaining essentials to community living: Identification, financial assistance

Arrange access to spiritual and cultural practices to support healing

Help maintain family contact during incarceration and in the community

Help facilitate recreational and cultural opportunities that include family

Arrange for addictions programming, housing, health, education/job skills and employment opportunities.

Participate in STR8 UP presentations

Provide information about services and opportunities in the community

Meet with team to help develop new initiatives and strengthen organizational strategies

Identify partnerships for services in the community

To qualify, you will possess or be working towards a degree or diploma in human services in any of the following fields: Social Work, Indigenous Social Work, Human Justice, Education, Arts: Justice Studies, Psychology, Sociology, Criminology, and Indigenous Studies, or relevant experience equivalent.

Salaries are based on STR8 UP current salary grid $25.00 to $30.00 per hour
