Closing the Gap Healthcare is seeking a** Full
- Tim**e** Care Team Coordinator (CTC) **to provide support to our team in Halton. The CTC’s role is an integral contribution to the quality of care provided by CTG. You will be coordinating client referrals with our team of Providers, providing empathetic client service and the administrative duties.

**The role will require**:

- A passion for challenges and working with team members to ensure a great experience
- Accurately and efficiently assessing team requests and addressing them immediately or escalating as appropriate
- Strong problem-solving skills
- Supporting daily office operations and program leadership team
- Working effectively with other team members
- Effectively triaging calls to ensure effective call management
- Performing data entry requirements for all clients using the CTG computer system
