**CLINICAL CASE MANAGER JOB SUMMARY**

Turning Leaf Support Services’ Community-Based Treatment (CBT) department as well as the Intensive Home Supports (IHS) department is designed to provide person-centered supports to adults (or individuals transitioning to adulthood) who are living with intellectual challenge, developmental disability and/or mental illness.

Reporting to the CBT Senior Case Manager and Director of CBT, IHS Senior Clinical Case Manager and IHS Director for IHS related matters, the Clinical Case Manager is responsible for the operations of all services provided to all Turning Leaf participants within their assigned region (this may include Intensive Home Supports, Guided Living, Cluster or Community-Based Treatments contracts).

Specifically, the Clinical Case Manager is responsible for overseeing the treatment/support service, management and operation of the residential facilities, and ensuring that program objectives and individual support plans/contracts are delivered and developed and maintained according to organizational standards.  

**ESSENTIAL REQUIREMENTS**
- 18 years of age or older
- A current resume and cover letter
- Criminal Record Check (no older than six months)
- Adult Abuse Registry Check (no older than six months)
- Child Abuse Registry Checks (no older than three months)
- Valid C.P.R and First Aid Certificate
- A minimum of three years’ experience in providing service to intellectually challenged adults and those experiencing mental illness.
- Understand and incorporate the clinical literature pertaining to the population served.
- A valid driver’s license, current driver’s abstract, appropriate current vehicle registration, as well as access to a car.
- Completion of a Turning Leaf (Inc.) Orientation and Training package (as deemed appropriate by the COO of Community-Based Treatment)
- Post-secondary degree or certificate related to human services considered an asset.
- Three employment references.

**QUALITIES AND QUALIFICATIONS**

Turning Leaf Clinical Case Manager must possess the capacity to be aware of and control one’s emotions. Moreover, Clinical Case Managers must be able to handle interpersonal relationships judiciously, empathetically, and professionally while executing their responsibilities and providing Turning Leaf’s person-centered supports.

In addition, the Clinical Case Manager must possess:

- Strong attention to detail and organization
- Ability to handle multiple projects and priorities in a professional and timely manner
- Consistency with processes and daily tasks
- Self-starter and proactive approach in resolving problems and issues
- Excellent oral and written communication skills
- Ability to read, analyze and interpret general information and data
- Ability to interact with all levels of personnel.
- Treat all with dignity and respect, open and receptive to their needs
- Troubleshooting skills
- Ability to effectively present information in a clear manner, demonstrate procedures and respond to questions
- Ability to accommodate afterhours calls/needs to provide support on an as need basis
- Ability to understand and incorporate the clinical literature pertaining to the population int
