**_Link Program Coordinator_**

**Job Summary**:Provides leadership and supervision to Aunt Leah’s Link and Link Housing First Program, a unique program focused on providing a continuum of supports for youth aging out of government care with a focus on housing, preventing homelessness and improving quality of life with the goal of providing supports akin to what parented peers receive.

**Reports To**:Program Director

**Qualifications**:

- Post-secondary education in a related field and/or a relevant combination of education and experience
- 1 to 2 years’ experience in program development, implementation and supervision
- Advocacy background and training
- Demonstrated experience in supporting marginalized and/or multi-barrier persons

**Requirements**:

- Undergo & maintain a satisfactory Criminal Record Check & Local Police Check
- Valid Class 5 driver’s license and access to a vehicle
- Produce a clear Driver’s Abstract
- Proof of vehicle business insurance of $5 million liability
- First Aid - Level C w/ CPR
- Food Safe
- CPI - Non-violent Crisis Prevention Intervention Training
- Agrees and adheres to the Society’s Code of Ethics

**_Key Duties and Responsibilities: _**

**Program and Case management**:

- Directing staff in the creation and maintenance of participant plans and referrals that encourage and support ongoing housing, rehousing and transition from homelessness to housing
- Responsible for management of Federal funding from Reaching Home and ensuring housing supports through this funding are used to prevent new inflows of housing, rehousing supports and to move under-housed or unhoused youth into homes
- Responsible for management of BC Housing Homelessness Prevention Program (HPP) funding and providing rent supplements for former youth in care
- Identify and provide emergency crisis services as necessary; make immediate emergency assessments and respond according to accepted crisis intervention methods and techniques; coordinate safety plans and other services as appropriate
- Ensure staff have up-to-date knowledge of pertinent policies, standards and procedures and adhere to same; take immediate action to correct non-compliance
- Provide direction and/or directly intervene to resolve daily operational and high-profile participant relations issues
- Responsible for the coordination of the Link Facebook Page
- Plan and oversee weekly drop-in sessions for past youth and mothers requiring assistance
- Ensure follow up with youth and mothers are completed and tracked
- Ensure staff are documenting effectively and accurately
- Liaise with residential program coordinators to ensure a seamless transition between programs
- Liaise with other Programs Coordinators to provide employment and training opportunities for Link participants
- Liaise with Coordinators of SEFFY, Branches, Connections as well as Fraser Health Nurse Practitioner and Dan’s Legacy Program Director
