The Director of Quality and Integrated Risk is currently seeking a Patient Safety/Quality Improvement Coordinator. The purpose of the position is to coordinate and integratehospital-wide patient safety (PS) and continuous quality improvement (CQI) initiatives which include but may not be limited to managing and analyzing quality/performance indicators in compliance with Accreditation Canada (AC) standards, Excellent Care for All Act (ECFAA), and applicable regulations.
**Please Note: This position offers a hybrid work environment consisting of in-office and remote work. Office location is 500 Church St in Penetanguishene, ON.**
**Key Accountabilities**:
- Coordinating hospital-wide PS and CQI initiatives incorporating the use of performance/outcome indicators
- Updating and maintaining patient safety and quality improvement policies and procedures
- Collaborating with managers and the Decision Support Team regarding the collection and reporting of indicator data on a hospital-wide basis
- Conducting, analyzing and reporting patient/client incident data and/or quality performance indicators registered in the electronic database and making recommendations regarding ways to mitigate patient safety concerns through system changes and improvements and/or discussing and offering solutions when possible
- Maintaining and upgrading of incident/safety data collection software
- Assisting in the maintenance of patient complaint/safety incident data files in compliance with regulatory agencies while promoting maximum confidentiality
- Participating in the review of adverse events and quality of care concerns - facilitating root cause analyses, conducting investigations, monitoring action plans and improvement efforts
- Assesses the educational/training needs of staff and teams related to patient safety and continuous improvement principles, methods and tools, and service evaluation activities
**Requirements/Qualifications**:
- Degree in a healthcare discipline and Certification/diploma in Quality Assurance and Patient Safety preferred
- Knowledge of Philosophy, principles, methodologies and techniques of PS and CQI as well as service evaluation
- Relevant legislation (i.e. Excellent Care for All Act, Mental Health Act (MHA), Personal Health Information Protection Act (PHIPA),Public Hospital Act (PHA), Quality of Care Information Protection Act (QCIPA), etc.), regulated health professional standards and codes of ethical conduct
- Knowledge of Accreditation Canada processes and standards
- Knowledge of clinical practice and standards of patient safety in order to provide leadership with respect to the identification and resolution of such issues
- Proven analytical thinking, interpersonal, planning and organizational skills
- Leadership and facilitation skills
- Excellent verbal and written communication skills
