**Job Summary**:
The Home Support Worker (HSW) provides support to clients and families within the framework of VON policies, procedures, standards. The HSW collaborates with members of the health care team in meeting the needs of clients to attain an optimal level of independence and well-being**.**

**Key Responsibilities**:

- Performs tasks reflecting the needs of the client and their household’s safety, which may include personal hygiene care, assistance with activities of daily living, assistance with ambulation and transfers, preparation of nutritious meals, some light cleaning, laundry. Observes and listens to the client and caregiver to identify whether the family’s needs have been met
- Contributes to the development of the personal support care plan by reporting any observed changes or needs to the Manager or Team Lead
- Implements approved changes to the care plan under the guidance of the Manager/Team Lead
- Provides respite care to clients as required
- Completes daily call checks to maintain the effective and efficient operation of the home support services
- Organizes each day's activities, setting priorities for completion of client personal care and activities of daily living
- Functions and reports in accordance with VON's policies, procedures, practices and guidelines designed to promote and protect client safety
- Other duties as assigned

**Education, Designations and Experience**:

- High school diploma or GED
- Certificate in Homemaking, Home Support Worker, or Health Care Aide is an asset
- Standard CPR level C Certificate required
- Demonstrated experience providing support services within community healthcare
- Household management experience
- Experience working with elderly or ill individuals and/or children

**Skill Requirements**:

- Strong verbal and written communication skills in a manner that demonstrates respect, empathy and understanding to clients, families and colleagues
- Good assessment skills
- Ability to work effectively in both a multidisciplinary team and independently
- Ability to problem solve
- Ability to take direction well
- Ability to work in various social and economic environments
- The capacity to adhere to strict confidentially

**Other**:

- Must have a personal vehicle for work and possess both a current driver’s license and proof of vehicle insurance
- A current and original copy of a satisfactory Criminal Records Check
- A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required
- Must be able to wear Personal Protective Equipment (PPE) for extended periods

Work Conditions and Physical Capabilities: Attention to detail; Lift and carry using appropriate lifting techniques; Bend, push, pull, grip, reach, kneel, crouch using good body mechanics; Walk, sit, stand, climb stairs; Hand movements.
