**Overview**
We are seeking a dedicated Dietitian for consulting to one Long Term Care Home to cover a maternity leave, working 2 days/week.
**Responsibilities**:
- Completing admission, quarterly and annual assessment and completing RAI-MDS
- Monitoring and adjusting residents plan of care
- Collaborate with healthcare professionals to ensure optimal nutrition care and quality of life
- Attending regular meetings for care conferences, PAC and CQI, wound care and other meetings as required
**Qualifications**:
- Registered with the College of Dietitians of Ontario
- Liability Insurance
**Job Type**: Fixed term contract
Contract length: 8 months
Pay: $1,800.00-$2,300.00 per month
