The Personalized Community Care (PCC) team works in partnership with Children’s Services and Collaborative Service Delivery agencies to provide 24-hour support to youth in a home environment. Youth involved with SPS generally require a level of support beyond that of a typical placement, such as in a foster home or group care setting.**Responsibilities**:_

Child & Youth Relationship:

- Create a safe and positive environment where youth can learn and grow
- Support youth to engage with their culture, community, family, and peers through role modelling and relentless support.
- Provide transportation for youth, enabling them to engage in community and family events, as well as attendance at school and professional support appointments
- Modelling, and teaching a variety of life skills including positive relationship building, appropriate boundaries, self-care/hygiene, and communication skills
- Support individuals to function more independently
- Teaches and assists individuals with activities such as hygiene, basic cooking, money management, shopping, and safety skills.
- Facilitates physical, recreational, social, and vocational activities
- Meet the educational needs of person(s) served (assistance with homework, day programming when person(s) served is not in school)
- Maintain consistency of house rules and routines, and consequences
- Protect resident privacy and information
- Provides feedback and support to individuals
- Recognizes, analyzes and deals with potential emergency situations such as individuals’ aggressive behavior to minimize potential harm to the person and/or the public
- Report issues and problems to the Supervisor
- **House Responsibilities: **_
- Light cleaning and cooking duties
- Maintain both the inside and outside of the facility
- prepare and serve meals, and carry out household cleaning duties
- Administers medication to individuals in accordance with established policies and procedures
- Facilitate effective shift exchanges, staff communication, and/or communication of issues or concerns
- Work in collaboration with the CSD Service Team and make recommendations based on observations and time spent with the youth
- Advocate and support youth to advocate for themselves to access resources or supports to enhance their connections and development
- maintains liaison with other agencies, professionals, government officials and the community
- Contribute to team effectiveness through clear and direct communication and participation in staff meetings
- Attend all training
- ** Documentation**:_
- Document observations and share with the Specialized Program Support (SPS) team at monthly meetings, focusing on strengths of youth
- Complete and maintain documentation of daily activities and observations.
- Participate in the Care Team through the ongoing development of care plans and programs
- Participates in assessment, goal setting and program planning such as Individualized Support Plans for persons supported.
- Documents and implements the plan.
- Provides input into the evaluation of the program.
- Evaluates individuals’ needs and develops short term and long-term plans to meet such needs with the active participation of the individuals and their personal networks (both professional and nonprofessional)
- Provides written and/or verbal reports regarding individuals’ daily activities and progress
- Ensures that all required documentation is complete and accurate.
- As a point person/key worker:_
- Assist the Supervisor with any additional duties as requested (which may include but not limited to cover off for holidays, support other staff members, ensure that all shifts are covered)
