The Family Wellness Connector plays a key role in delivering services through our new Family Mental Wellness Hub. This multifaceted position blends system navigation, crisis intervention, group facilitation, and community engagement to support families navigating mental health, addiction, and social service systems. You’ll act as a trusted first point of contact for families, coordinate referrals, support service planning, and build strong partnerships across agencies.

We are seeking a self-motivated, optimistic, and adaptable individual who is excited to shape and grow a new program in a team setting.

**LOCATION**: Child and Family Centre, 1804 Home Road NW

**HOURS**: Full time, 36 hours per week, typically Tuesday to Saturday, or Sunday to Thursday, 9 to 5 with ability to adjust based on program needs at the discretion of the program manager.

**SALARY**: $56,000 - $60,000 per year

**RESPONSIBILITIES**:
**Direct Client Work**
- Act as the initial point of contact for families seeking support through the Family Wellness Hub;
- Provide compassionate, non-judgmental, and client-centered support to individuals and families navigating mental health and addiction challenges;
- Assess family needs and coordinate referrals or match them to appropriate internal mental health service providers, and support the development of strength-based service plans
- Maintain ongoing communication with families, providing follow-up, advocacy, and encouragement throughout their care journey;
- Offer immediate resources and intervention strategies to prevent crisis situations and support stabilization;
- Plan, prepare, and facilitate psycho-educational, support and/or peer groups to address common needs and foster a sense of connection and community among families;
- Record all client interactions, group activity, and service referrals in accordance with documentation standards and privacy policies.

**Community Engagement**
- Build and maintain strong relationships with external community partners, agencies, and service providers to expand available support options.
- Provide outreach and co-location support to community partners seeking services for mutual clients, enhancing collaboration and care continuity.
- Represent the Family Mental Wellness Hub, and agency programs, at engagement events, community meetings, and partnership tables;
- Identify gaps in service access and advocate for innovative collaborative solutions;
- Share program information and promote awareness of the Hub within diverse communities through intentional outreach and relationship-building efforts;
- Ensure families are connected to culturally relevant and community-informed services.

**Internal Agency Support**
- Collaborate with internal program teams to align family needs with available services to ensure timely coordination of care;
- Coordinate and facilitate agency-wide case conferences to support the daily work of frontline staff and promote collaborative problem-solving.
- Build and maintain an up-to-date database of mental health resources for staff and client use.
- Provide regular feedback on trends, challenges, and emerging needs observed through client interactions;
- Participate in staff meetings, supervision, and professional development activities as required;
- Support data collection and reporting related to group outcomes, referrals and service utilization;
- Participate in other duties or training as directed by the Program Manager, Director, or CEO.
