**Currently Seeking**:(1) **In Home Personal Support Worker (PSW/DSW related)

Seeking a caring, reliable, enthusiastic, friendly and trustworthy individual to provide PSW services to an individual living with dementia. **This is a Part time (10 hr) sleep position.**_(some flexibility with time may be required)_
- _**Must be available to work every other weekend (rotating with f/t sleep staff)**_
- Must have a valid certification in a form of Home Health care or related field (PSW, CSW, DSW, SSW) and have a minimum of 1 year experience.
- Must be able to collaborate and work harmoniously with caregivers, staff and other agencies, under the direction of Management.
- Personal Care assistance with client required
- **Must be physically fit to complete daily tasks such as rotating, adjusting, transferring and pivoting clients with mínimal supervision.**
- Participate and collaborate with other health care providers in the provision of client care.
- May be required to administer medications, under the direction of caregivers
- Carry out all work assignments safely and be knowledgeable of and in compliance with LSS guidelines, regulations, policies and procedures.
- Remain in compliance with Occupational Health and Safety Act and Human Rights Services and its regulations.
- Complete all daily paperwork such as; client communication notes, incident reports (when required), Injury Reports (when required) and any other necessary communication notes.
- Must complete yearly OSHA requirements and WHIMIS Certifications.
- Assist with personal care activities
- Abide by LSS’s Support Plan and structures in place by caregivers
- Ensuring the safety of clients and fostering a safe and supportive environment

**CONDITIONS OF EMPLOYMENT**

1. Must comply with: Leslie’s Support Solutions and Human Resource Policies and Procedures

2. Must Obtain satisfactory Vulnerable Sector Check

3. Must provide and maintain Personal Support Workers License

4. Must have a valid First and and CPR certification

5. Must be able to work the hours provided by the caregiver

6. Must provide a valid form of identification

7. Must be comfortable administering medications

8. Must be Caring, compassionate and physically fit

9. Must remain in good standing with the company and caregivers.
