Social workers in health care provide in-depth bio-psychosocial assessments, diagnostic, treatment, best practice based interventions and evaluation for individuals and families. Counseling, discharge planning, resource referral, consultation and collaboration, teaching, research and patient/family education are all part of the social work role.

The St. Joseph’s Health Centre Outpatient Oncology clinic provides comprehensive outpatient cancer care. The Social Worker works within a specialized interprofessional team dedicated to quality cancer care to support patients through diagnosis, treatment and follow-up.

**DUTIES & RESPONSIBILITIES**:

- Able to work effectively as a member of an inter-professional team, collaborating with other disciplines to provide comprehensive patient care;
- Willing to contribute to the academic mandate of the organization as an educator, researcher, resource or support for staff, students and members of the academic and professional community;
- Identifies and addresses educational needs for patients and their families;
- Demonstrates commitment to ongoing professional development in order to continue to provide optimal patient care;
- Is committed to, and participates in, ongoing quality improvement in the organization;
- Provides patient care in accordance with all professional and organizational standards;
- Demonstrates understanding of the impact of the social determinants of health and has a strong knowledge of how to access and link patients to appropriate community resources and services such as: OW, ODSP, Trillium, CAS, Community Housing, HACC etc.
- Skilled in biopsychosocial assessment, crisis assessment and development of treatment recommendations.
- Ability to manage high volume of referrals, prioritize and broker to appropriate resources and bridging where appropriate.
- Working closely and in partnership with the Oncology Physician and Clinical Care team, this role will provide counselling services, education, and community referrals, while working collaboratively with patients, families, internal and external partners.

**QUALIFICATIONS**:

- Must have a Master's Degree in Social Work (MSW) or Equivalent;
- Must be currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for practice in all applicable clinical areas;
- 2 years’ experience with the provision of psychosocial counselling within the Oncology patient population preferred
- Must have experience working in an acute care setting with knowledge and skills necessary for system navigation.
- Experience working with populations who may experience marginalization.
- Experience providing anti oppressive, anti-racist, accessible care for all patients.
- Current CPR certification;
- Excellent communication, critical thinking and interpersonal skills;
- Proven flexibility and adaptability in a fast-paced environment;
- Excellent time management, organizational and prioritization skills as relates to patient care

Staff must be available to work all shifts including weekdays/weekends based on program and department needs.
