**Job Summary**:
The Adult Day Program (ADP) -Activation Support Staff will be responsible for the implementation of group as well as one-on-one activities within our Adult Day Program for persons living with dementia. You will be responsible for ensuring clients receive physical, social, and cognitive stimulation through the use of games, sports, hobbies, crafts and more.
The Activation Staff will be a part of the client’s circle of care and will support in:
- Maintaining clients physical and emotional well being.
- Provide support in developing and facilitating recreational activities throughout the day.
- Evaluate clients at specified intervals to ensure they are receiving appropriate dementia care in group activities.
**Knowledge/Skills/Abilities**
- Experience working with persons living with dementia in group settings or one-on-one.
- Experience designing recreational programs to support individual needs as well as facilitating group activities.
- Knowledge of community health care agencies.
- Current Standard First Aid/CPR certification.
- Ability to work cooperatively with volunteers, staff, and community partners.
- Excellent communication (verbal and written)
- Exceptional interpersonal skills, including shared decision-making and facilitation
- Ability to take initiative and be resourceful
- Excellent problem-solving and change management skills
- Proficiency in technology (e.g.: Microsoft office and case management and care coordination systems)
- Demonstrated ability to work independently and within a team
- Expertise and experience in cultural sensitivity and diversity
**Responsibilities**:
- Facilitate activities for persons living with dementia.
- Follow activation calendar and be able to assess client’s eligibility and appropriateness for recreational activities.
- Effectively participate in care conferencing.
- Follow established job routines on a daily basis.
- Ensure documentation is performed in an efficient and timely manner.
- Work within our outreach adult day program when required.
**Other Duties**
- Assume other functions as may be assigned by the Program Manager
**Job Qualifications**
- Post-secondary degree or diploma in Recreation Therapy, Gerontology, or Health Promotion. Other health disciplines may be considered given appropriate combination of education and experience
**Other Requirements**:
- Bilingualism (English/French)
- Ability to travel within chapter district as required
- A valid driver’s license and access to a reliable vehicle is required
- Criminal Records Verification (within 2 months), incl. Vulnerable Sector Screen
- Must be able to provide a negative 2-step TB Test

**Emplacement principal : Sturgeon Falls**
La préposée ou le préposé à l’activation est responsable de la mise en œuvre d’activités individuelles et de groupe dans le cadre de notre Programme de jour pour adultes vivant avec une démence. Au moyen de jeux, de sports et d’activités d’artisanat, entre autres, cette personne s’assure que les clients sont stimulés sur le plan physique, social et cognitif. Elle fait partie du cercle de soins des clients et contribuera à:
- maintenir le bien-être physique et émotionnel des clients;
- créer et animer des activités récréatives pendant la journée;
- évaluer les clients à intervalles précis pour s’assurer qu’ils reçoivent les soins appropriés propres à leur démence pendant les activités de groupe.
**Connaissances, compétences, expérience**:
- Expérience de travail auprès des personnes vivant avec une démence, en groupe ou individuellement.
- Expérience de la conception de programmes récréatifs qui satisfont aux besoins individuels et de l’animation d’activités de groupe.
- Connaissance des organismes de santé communautaires.
- Certificat valide de secourisme général et en réanimation cardiore
