**Care Coordinator - Belleville Physiotherapy & Sports Injuries Clinic**

**Qualifications & Core Competencies**:

- High School Diploma, G.E.D. or equivalent
- High degree of organizational and wareness of AODA
- Required 1-5 years experience performing a similar role
- Excellent communication and interpersonal skills
- Computer savvy and data entry skills in a setting working with vulnerable people
- Flexible and adaptable to change
- Ability to hit the ground running with minimum supervision

**Primary Responsibilities**
- Answer telephone calls, greet applicants and visitors upon arrival, and provide excellent customer service for all questions or concerns
- Provide information to applicants on the phone and walk-ins
- Maintain contract timelines and communicate effectively with external customers
- Provide administrative support to the Application Eligibility Team of Occupational Therapists
