**Position Summary**

The Family Support Worker (FSW) is a member of the Healthy Growth and Development team which is responsible for the delivery of the Healthy Babies Healthy Children Program as mandated by the Ontario Public Health Program Standards 2018. Under the direction of Program Manager and in collaboration with the Public Health Nurse, the FSW works with families to: develop parenting skills and knowledge, assist families with accessing community resources and services, and work to meet the identified family goals.

**Qualifications, Ability, Experience, and Skills for this position include (but not limited to)**:

- Minimum grade 12 diploma;
- Demonstrates knowledge of strategies that support families in developing positive parenting skills;
- Experience working with families with young children, an asset;
- Extensive knowledge and the ability to navigate and link clients to community resources and organizations;
- Ability to collaborate and communicate effectively with diverse individuals, families, health and social services professionals;
- Ability to effectively work independently or as part of a multidisciplinary team;
- Use of a reliable automobile, valid “G” Ontario Driver’s Licence and ability to travel is a requirement;
- Strong computer skills and an ability to learn new computer programs are essential;
- The ability to communicate (verbally & in writing) in French or any other language(s) will be considered an asset;
- Strong interpersonal, organizational and problem-solving skills;
- Must be prompt, reliable and maintain a good attendance record;
- Ability to work outside regular business hours, including weekends and evenings, as required;
- A current Police Vulnerable Sector Check (PVSC) with satisfactory clearance is a condition of employment, at own expense.
