Are you passionate about providing case coordination and management to the clinical health team to support client care needs and navigates service gaps for children and youth? We're seeking a dedicated individual to join our team as a Health Services Clinical Coordinator. The Clinical Coordinator focuses on coordinating discharge planning, collaboration with interdisciplinary care teams, and health programming specific to the development of improved systems. This role requires a deep understanding of Indigenous cultures, healthcare systems, and remote service delivery methods. The role will report to the Associate Director, Clinical Services.

**Responsibilities**:

- Completes client assessments for the integration of service providers into the plan of care
- Advocates for the inclusion of cultural protocols and traditional healing practices in the delivery of mental health services, ensuring indigenous patients have access to the resources and supports that align with their cultural beliefs and practices.
- Advocates for the continuity of care and access to necessary mental health services, ensuring that indigenous patients' mental health needs are met in remote community and urban settings.
- Researches and identifies appropriate community resources and services that can support indigenous patients upon their return to the community, such as home care, mental health services, social support programs, and cultural revitalization initiatives.
- Provides direction, education, and guidance to health care providers and staff, to ensure quality care is provided through a family-centered lens.
- Provides supervision and leadership to positions pertinent to supporting the best practices in Indigenous health care, ensuring maintenance of best practices in clinical care, and implementation of specialty health services.
- The Clinical Coordinator will supervise the Staff Educator, Reclaiming Our Bundle Lead, and Medical Office Administrators.
- Creates and maintains client care plans and documents according to the OCSWSSW.

**Position Requirements**:

- Bachelor’s degree in nursing, social work, medicine, occupational therapy, physiotherapy, or related health services and in good standing with regulatory college or professional association OR Community and Health Services Navigation College Certificate.
- 2 years recent related experience in a healthcare, or a combination of related education, training and experience.
- Experience in an Indigenous health and or social service organization, hospital or community-based case management, health promotion, public health or social work
- Familiarity with health services within the region and knowledge of Indigenous-specific considerations in care coordination.
- Proficient in the use of Microsoft Office Suite and scheduler platforms.
- Clear Background and Vulnerable Sector Check and proof of immunizations (or valid medical exemption).
- Maintain up to date on clinical practise standards.
- Ability to maintain a high level of confidentiality when handling sensitive information.
- Ability to travel to IFNA communities via small aircraft or vehicle.
- Problem-solving, good judgement and decision-making skill and ability to stay calm and composed especially in times of crisis or urgencies.
- A good understanding of the culture and the language is a definite asset.
- Familiarity with the independent First Nations Alliance First Nations is also an asset.

**Salary Range**: $75,000 - $86,820

**Schedule**: Regular hours from Monday to Friday with weekends, overtime and on-call availability.

Pay: $75,000.00-$86,820.00 per year
