**Job Summary**
The Clinical Care Coordinator plays a crucial role in ensuring the smooth operation of services within our facility. This position involves coordinating patient care, managing records, and facilitating communication between patients and healthcare providers. This position would serve as a key pillar in ensuring that our mental health services remain accessible, organized, and client-centered as our programs continue to grow.
**Key Responsibilities**:
- Manage the clinical intake process and coordinate client-clinician matching.
- Maintain communication with clients pre
- and post-therapy, ensuring follow-up, system navigation and payments.
- Provide administrative support to social workers and Director-: scheduling, progress tracking, and coordination with external supports.
- Track client outcomes and service usage to assist with reporting and strategic planning. Maintain accurate, confidential records in coordination with clinicians.
- Follow up with therapists to ensure payments are made in a timely manner
- Support the AMRC clinical team and reflect a commitment to holistic, client-centered care that addresses the full scope of a person’s needs.
- Work closely with other AMRC staff to complement other services
**Qualifications**:
- **Knowledge of Resources**:in and outside of the Muslim community.
- **Community Education and Experience**:
- Post-secondary education in **Social Work**, **Psychology**, **Health Administration**, or a related human services field.
- Knowledge of **trauma-informed**, **client-centered**, and **culturally responsive** care models.
- **Administrative and Organizational Skills**:
- Proven ability to manage **scheduling**, maintain **confidential records**, and support **case coordination**.
- Strong **organizational and time-management skills**, with attention to detail and the ability to prioritize competing demands.
- **Communication and Interpersonal Skills**:
- Excellent verbal and written communication skills; ability to build trust and rapport with clients and professionals.
- Demonstrated ability to work effectively with **diverse populations**, including newcomers, racialized communities, and individuals experiencing trauma or mental health challenges.
- **Technical Proficiency**:
- Comfortable using **electronic health records (EHR)**, **Microsoft Office Suite**, and **cloud-based tools** for scheduling, reporting, and communications.
- **Professionalism and Ethics**:
- Understanding of **confidentiality**, **privacy laws**, and **ethical boundaries** in clinical and social work settings.
- Ability to work both independently and collaboratively as part of an **interdisciplinary team**.
- **Additional Assets**:
- Experience in **system navigation**, **intake coordination**, or **case management**.
- Experience in **clinical administration**, **mental health services**, or **community-based health programs** is strongly preferred.
- Familiarity with **community mental health services**, **settlement services**, or **refugee support systems** is an asset.
- Ability to communicate in additional languages is an asset.
