**Location**: Hybrid, with on-site work at 145 Front Street and at shelter sites/other ICHA-supported locations
**Role Description**:
Reporting to the Population Health Program Manager and working closely with all Population Health Services (PHS) team members, the Health Assessor will provide administrative, organizational, and data collection support to the Enhanced Community Assessment and Risk Evaluation (CARE 2.0) program at ICHA. They will be responsible for various tasks including documenting the support and medical risk scores associated with the project and developing guidance documents with the PHS team and other project stakeholders.
**Roles and Responsibilities**:
- Contacting assigned shelter and drop-in sites to retrieve client risk scores related to medical and support needs and troubleshooting related to this activity.
- Documenting and organizing results in secure files/folders.
- Keeping data accurate for regular review and evaluation and contributing to project guidance documents.
- Offering trainings to shelter staff and partners on population health assessment tool utilization.
- Offering surveillance health assessment for people experiencing homelessness using the population health assessment tool.
- Supporting the Data Analyst and others with the dissemination of CARE-2.0 evaluation findings, tools, and data dashboards nationally and internationally through open-access publication.
- Responding to enquiries and troubleshooting data dashboard issues along with other team members.
- Providing access or mediating access to data dashboards as appropriate.
- Supporting initiatives to automate data collection processes and to enhance data procedures.
**Qualifications**:
- Postsecondary education at a bachelor's level or 3 years of equivalent experience in data collection, analysis, assessment.
- Excellent computer skills (including Microsoft Office) and an ability to learn new systems.
- Knowledge and experience using Excel and working with data.
- Experience with marginalized populations with complex health and social service needs is highly desired.
- Open minded, non-judgmental with a commitment to upholding the rights and dignity of all people and respecting their diversity.
- Ability to work effectively within an inter-professional team.
- Solid organizational and time management skills; ability to support and communicate with multiple shelter sites concurrently, continuously identifying and addressing competing priorities in a timely and effective manner.
- Excellent verbal and written communication skills.
- Excellent attendance and punctuality record.
